As biological therapies continue to evolve as valuable tools in the multimodal management of canine osteoarthritis (OA), so too does the debate pertaining to the cost:benefit conundrum. To support clinical decision-making, below is a concise evidence-based summary of Stem Cell Therapy using mesenchymal stem cells (MSC) therapy

Primary action: Regenerative + immunomodulatory
Tissue impact: Can differentiate into chondrocytes; chondroprotective
Best for: Moderate–severe OA, chronic cases, NSAID-intolerant patients
Duration: 6–18 months or longer
Dosing: Single / infrequent
Safety: Good in controlled studies
OA Role: Moderate–severe OA or refractory cases

Key Clinical Evidence in Dogs
MSC Therapy
Improvement in pain and lameness in prospective controlled trials
Harman et al., 2016
Sustained benefit reported up to 6–18+ months
(Kriston-Pál et al., 2020; Olsson et al., 2021)
Immunomodulatory effects demonstrated (reduced synovial inflammatory mediators, systemic immune activity)
(Beerts et al., 2023)
Most benefit seen in **moderate-to-advanced OA**
* Useful when NSAIDs/injectables insufficient or contraindicated
* Single or limited treatment sessions; allogeneic and off-the-shelf use supported
(Luo et al., 2021)

Mesenchymal stem cells (MSCs) are a popular option when it comes to reducing inflammation and promoting healing in horses, as they carry a very low risk of side effects compared to other types of stem cells. While MSCs were originally used mainly for treating joint and muscle problems, they are now being tested for a variety of other health issues in horses, offering promising new treatment options. Osteoarthritis (OA) is a degenerative joint disease with a high prevalence in dogs. Mesenchymal stem cells (MSCs) have been used to treat humans, dogs, and horses with OA.In a recent, randomised, masked and placebo-controlled efficacy study it was determined that allogeneic adipose- derived stem cell treatments were effective in treating dogs with OA.
