Looking to use CareCredit as a form of payment for your pet’s next visit?

❗️For every person using a CareCredit card we MUST have this information on file AND update this information every 4 years❗️

• The person who is named on the card must be physically present.
• The physical card must be present.
• We must see and photocopy a photo ID such as ( Driver’s License/State ID )
• If you are not the cardholder we must have a signed document from the cardholder granting permission to the user. This document must have the date range for authorized user. (This cardholder must have already physically presented the card and photo ID).

We encourage all clients to email or provide this information PRIOR to your pet’s visit!😀
