A hospital must be legally prohibited from discharging any patient with an extensive mental health history into homelessness. If a patient requires community management, the NHS and the housing authority must co-sign a unified, self-contained accommodation plan before discharge.

3. EMERGENCY DISCRETIONARY HOUSING UPLIFTS: Create a fast-tracked, emergency top-up fund within local infrastructure budgets that instantly bypasses standard Universal Credit delays, directly covering private sector deposits for vulnerable individuals caught in systemic loops.
