1. Pet name & breed – so I know exactly who’s on my table and what coat type or style works best.

2. Gender, age & weight – this helps me plan the right approach, since every dog has different needs depending on their size and life stage.

3. Vaccination date – keeps everyone safe and healthy during their visit.

4. Spayed/Neutered – sometimes this can affect behavior or handling.

5. Behavior notes – every pup has their quirks! Knowing if they’re shy, playful, or nervous helps me make them feel at ease.

6. Last grooming visit – tells me how much work might be needed and how to make the session smoother.

7. Health issues – very important for safety! If your pup has skin sensitivities, allergies, or joint problems, I can adjust my tools and handling.

8. Vet & emergency contact – just in case, I want to be prepared for your dog’s wellbeing.

9. Pet image – a photo helps me remember your furry friend between visits. 🐶💛
