Renal calculi

4 tips to remember when you’re doing an ultrasound in a dog with suspicious of renal calculi.

1. Remember to obtain different views from the organ. The transversal view can offer better visualisation of the renal pelvis.
2. Using a high frequency probe in some small dogs and cats can offer a clearer image.

Pericardial and pleural effusion

Story from previous week.
Old cat with unspecific clinical signs (lethargy and anorexia mainly).

Opinions were shared between pericardial effusion or both (pleural and pericardial). It’s true that the first clip only allowed to see pericardial effusion, however if we see the clips at the end we can see the limits of the pericardium and also fluid on the pleural space.

In this clip you can appreciate how the volume of the pericardial fluid disappears while we perform pericardiocentesis. At the end of the video, only pleural effusion is present.

Soft tissue sarcoma

An adult dog presented with a focal hard swelling on the left side of the neck.

Ultrasound scan showed an irregular, hypo echoic structure located between the cervical muscles.
The lesion was heterogeneous and highly vascularised when assessing under microvascular doppler.

FNA was obtained and the cytology exam was consistent with soft tissue sarcoma. The lesion was surgically excised and sent for histopathology.

Medial iliac lymph nodes

The medial iliac lymph nodes (MILN), internal iliac (hypogastric) and sacral nodes form the iliosacral lymph center. The MILN are located at the level of the aortic trifurcation, between the deep circumflex iliac and the external iliac arteries.

The MILN are usually the largest of this group, and are easier to identify.

These nodes drain mostly caudal structures (urinary bladder, prostate, v***a, te**is, re**um, anum, a**l sacs, hind limbs, tail…).

These lymph nodes belong to a dog which presented with a swollen a**l sac. His vet suspected about a**l sac adenocarcinoma and referred him for abdominal scan, looking for possible lymphadenopathy.

Unfortunately, in this case the MILN were severely enlarged, hypoechoic, rounded ( short:long axis >0.5), with peripheral vascular pattern. Fine needle aspiration (FNA) was performed, and the cytology was consistent with metastatic node from a**l sac adenocarcinoma.

Case from last week:
a) free fluid
b) splenomegaly
c) acute pancreatitis ✅
d) neoplasia

The clip shows an irregular and hypoechoic structure in close relation with the descendent duodenum, consistent with an enlarged pancreas. The surrounding mesenteric fat is hyperechoic, suggesting fat saponification or focal peritonitis.

The most likely diagnosis in this case would be acute pancreatitis, however, it’s important to remember that pancreatitis cannot be differentiated from pancreatic neoplasia based on ultrasound appearance. Therefore, follow up scan should be recommended in 1-2 weeks or sooner if clinical signs don’t improve despite medical treatment.

This dog responded well to the medical treatment.
