CASE OF THE DAY: “What’s Got This Pup Pawing at His Face?” 🐶🔍

History:
4-year-old, “Gator,” has been pawing at his mouth for a couple of days.
• Owner’s fear: “He was chewing on my sewing pincushion last week — is there a needle stuck in there?!” 😱
We’re on high alert for a foreign body emergency. Let’s investigate…

👀 Intraoral Exam Under Sedation:
Mouth propped open… and there it is.
BUT NOT A NEEDLE.
GUESS WHAT WE FOUND? 👇

[📸 IMAGE 3 and 4: Post-removal – inflamed gums where stick fragments was stuck….no needle in sight!]

[📸 IMAGE 5 and 6: abscess forming on lateral side of upper fourth premolar – directly opposite of where the stick was lodged. Plus is accumulating under the gum line. A needle was used to lance that pocket to allow the pus to drain.

✅ Diagnosis:
Stick Impaction Syndrome (yes, it’s a thing!)
The stick splintered from yard play days… not the pincushion incident — and got perfectly jammed between Gator’s upper PM4s.
The pincushion? Just a red herring. 🧵 (Thankfully, no needles!)

⚠️ Why This Happens:
• Upper 4th premolars are the main chewing teeth in the mouth. The power of a large breed dog’s bite can often fracture both side of the stick simultaneously, causing the stick to get lodged in between those teeth.
• Pressure → pain, swelling, drooling, infection risk.
• Owners often blame the last thing they saw (pincushion, bone, toy), but the real culprit is often nature’s toothpick.

🛠️ Treatment:
• Anesthesia + dental probe
• Gently dislodged the stick (it was really stuck!)
• Flushed with antiseptic
• Pain meds + antibiotics
• Gator ate dinner like a champ that night! 🍽️
