CARING HANDS PET GROOMING BY LOUELLA DAVIS to be referred to as Caring Hands in this document.

Grooming Consent Waiver Form

Current Vaccinations/Veterinarian Information: By signing this contract, pet owners verify their pets are current on all age-appropriate vaccinations for rabies. Proof of vaccination, current titer testing, or a veterinarian letter stating reason for not giving vaccinations shall be provided to Caring Hands. Permission is also given for Caring Hands Pet Grooming By Louella Davis to access veterinarian records for 1 year after last groom visit, this is permission for Caring Hands to talk to the veterinarian and support staff and also to receive copies of health records to confirm all vaccinations, current health status, and any information needed related to any pet bite. If your pet were to bite a person or another animal and they are not up to date on vaccinations, animal control will be notified and they may quarantine your pet.

Health Conditions & Senior Pets: Grooming procedures can sometimes be stressful, especially for a senior pet or pet with health conditions. Grooming may expose hidden health conditions or aggravate current ones during or after the groom. Senior pets and pets with health conditions have a greater chance of injury; these pets will be groomed for cleanliness and comfort, in styles that will not add to their stress.

Accidents: Every effort is made to ensure that your pet is groomed as safely as possible, but there is always the possibility an accident may occur. Pets are wiggly and excitable at times. Grooming equipment is sharp, even though I use extreme caution and care, it is still possible to injure a pet. Injuries may include, but are not limited to; cuts, nicks, scratches, quicked nails, brush burn, clipper rash, skin sensitivities, etc.

Aggressive or Dangerous Pets: Owners MUST inform Caring Hands if your pet bites, has bitten, or is aggressive towards people, other pets, or specific grooming procedures. Muzzles may be used if necessary. Caring Hands reserves the right to refuse/stop services for pets at any time before or during the grooming process. A special handling fee may be charged, as it takes extra time and care to groom aggressive pets.

Parasites: I do not accept pets with known fleas. Please let me know if you are aware or suspect that your pet has ticks at check-in. If fleas are found before bathing starts you will be required to come back and take the pet away without delay. If fleas or ticks are found during the grooming process, Caring Hands will use a flea and tick shampoo to remove the parasites and a fee will be added.

Mat Removal: We will discuss options for matted pets at check-in. Mats left in a pet’s coat only become tighter and can cause skin irritation and pain, as well as other health conditions. Caring Hands will brush/comb out minimal mats in a coat if it can be done safely and without causing discomfort to the pet. Some matting cannot be brushed/combed out and therefore will need to be shaved. Removing a heavily matted coat increases the risk of nicks, cuts, abrasions, or hematomas. Heavy matting can also trap moisture/urine near the pet's skin, which causes skin infections and irritation. Skin irritations caused by matting may not be evident at check-in; once the coat is removed skin irritations are made visible. The after effects of mat removal procedures can include itchiness, skin redness, self-inflicted irritations or abrasions and failure of the hair to re-grow or change in color or texture, especially with repeated short shaves. Caring Hands is not responsible for conditions or injuries resulting from the removal of mats. Prevention is the best defense against matting by scheduling regular grooming appointments and daily brushing/combing routine at home. I required all dogs to be on a 12 week or less schedule to remand as a client.

Late Arrivals: I charge a late fee for any appointment that is 20 minutes or more late, per pet. If you are 30 minutes late, you will be considered a no-show, your appointment will be canceled and a same day cancellation fee will apply.

Late Pick-Ups: ​​All pets must be picked-up within 45 minutes after contacting you. Picking up after 45 minutes my incur a fee unless arrangements were agreed upon prior to appointment.

No-shows & Cancellations: No-shows and last-minute cancellations are detrimental to my business. Please be respectful of my time as I am a by-appointment business, and another client could have taken your appointment, had I known you were unable to make it. The fee is not punitive, but I don’t get full pay when appointments do not show up. Please let me know 24 hours before your appointment, to avoid being charged a cancellation/no-show fee. Repeated no-shows or late cancellations may incur the need for prepayment before booking or removal from client list.

Photography: Photographs or videos may be taken of your pet and used for advertising and social media.

This contract/agreement gives Caring Hands permission to obtain immediate veterinary treatment for my pet, should it be deemed necessary by Caring Hands. By signing I agree that veterinary care expenses will be covered by me the pet's owner. I am also verifying that my pet is free of transmittable diseases.

Caring Hands Pet Grooming By Louella Davis will do my best to contact the owner first, then take the pet to the nearest veterinarian that is available.

Hold Harmless Agreement: By signing this contract you (or your Agent) agree to hold Caring Hands Pet Grooming By Louella Davis harmless from any damage, loss, or claim arising from any condition of the undersigned pet, either known or unknown.

If there are any problems with your pet’s grooming, please make me aware of them within 48 hours and I will do my best to resolve the situation.

By signing below, I confirm that I have read and agree to the policies of Caring Hands Pet Grooming By Louella Davis and will be in effect for the life of my pet listed on the other side of this form.

Required Vaccinations
• Rabies: Required by state law; initial vaccines are usually boostered 1 year later, then every 1 to 3 years depending on the vaccine used. (I require vaccination every 5 years after the initial 2 shots or titer test every year starting 5 years after the initial 2 shots or veterinarian letter stating reason for not vaccinating.)

• Non-Required Vaccinations but suggested
• DA2PP (Distemper, Adenovirus-2/Hepatitis, Parvovirus, and Parainfluenza): Protects against severe, highly contagious, and often fatal viruses.
• Bordetella (Kennel Cough): Highly recommended and typically required by groomers, dog parks, and boarding facilities in Minnesota.
• Lyme Disease & Leptospirosis: Highly recommended for dogs that hike, camp, or spend time near the heavily wooded and river areas common in Minnesota.
• Canine Influenza: Recommended if your dog frequently interacts with large groups of other dogs at doggy daycares or training classes.
• FVRCP (Feline Distemper): A core vaccine considered the standard of care by local veterinarians. It protects against Feline Viral Rhinotracheitis, Calicivirus, and Panleukopenia.
• FeLV (Feline Leukemia): Highly recommended for kittens and essential for cats that go outdoors or live in multi-cat households.
