Our Story
Services Primary Home Care (PHC) Program This program is a nontechnical, medically related personal care service provided to adults whose health problems cause them to be functionally limited in performing activities of daily living, according to a statement of medical need. PHC provider agencies also provide Family Care services. This service also is a nonskilled, nontechnical service provided to eligible clients who are functionally limited in performing daily activities. Covered services Es**rt: Attendants accompanying clients on trips to obtain medical diagnosis, treatment, or both. This service does not include the direct transportation of the client by the attendant. Home Management: Assistance with housekeeping activities that support the client’s health and safety, including: -changing bed linens -housekeeping -laundering -shopping -storing purchased items -washing dishes Personal Care: Assistance with activities related to the care of the client’s physical health, including: -bathing -dressing -grooming -routine hair and skin care -preparing meals -feeding -exercising -helping with self-administered medication -toileting -transferring/ambulating Community Attendant Services (CAS) Program CAS is a non-technical, medically related personal care service that is available to eligible adults and children whose health problems cause them to be functionally limited in performing activities of daily living according to a practitioner's statement of medical need. Services are provided by an attendant. Services Es**rt: Accompanying the client on trips to obtain medical diagnosis, treatment, or both. Additional time may not be allocated for es**rt services for other purposes. This service does not include the direct transportation of the client by the attendant. Home Management: Assistance with housekeeping activities that support the client's health and safety, including: -changing bed linens -housekeeping -laundering -shopping -storing purchased items -washing dishes Personal Care: Assistance with activities related to the care of the client's physical health, including: -bathing -dressing -grooming -routine hair and skin care -preparing meals -feeding -exercising -helping with self-administered medication -toileting -transferring/ambulating Family Care (FC) Program Family Care is a nonskilled, nontechnical attendant care service for eligible adults who are functionally limited in performing activities of daily living. Primary Home Care provider agencies have the option of providing these services. Family Care services are provided by an attendant and do not require the supervision of a registered nurse. Services Es**rt: Attendants accompany clients on trips to obtain medical diagnosis, treatment or both. This service does not include the direct transportation of the client by the attendant. Home Management: Assistance with housekeeping activities that support the client’s health and safety, including: -changing bed linens -housekeeping -doing laundry -shopping -storing purchased items -washing dishes Personal Care: Help with activities related to the care of the client's physical health, including: -bathing -dressing -grooming -routine hair and skin care -preparing meals -feeding -exercising -helping with self-administered medication -toileting -transferring/ambulating Personal Care Services (PCS) PCS is a non-waiver Medicaid benefit which provides attendant care services to individuals under 21 years of age who require assistance with activities of daily living because of a physical, cognitive, or behavioral limitation related to their disability or chronic health condition.
