As a general practitioner, your ability to recognize when a case has exceeded the scope of routine management is one of the most consequential clinical decisions you make. In ophthalmology, the window between "treatable" and "irreversible" can close quickly.

- Acute glaucoma with elevated IOP and ocular pain;
- Sudden vision loss or complete blindness of unclear etiology;
- Penetrating trauma or severe ocular injury;
- Rapidly progressive or melting corneal ulcer;
- Proptosis or acute exophthalmos.

🟑 Urgent β€” refer within days

- Anterior uveitis with undetermined or unresponsive etiology;
- Mature or hypermature cataract, particularly when lens-induced uveitis is suspected;
- Ocular pain or discharge not resolving after 48–72 hours of appropriate treatment;
- Suspected SARDS or IMR β€” early-stage disease carries a significantly better treatment prognosis;
- Non-healing corneal ulcer requiring debridement, keratotomy, or surgical grafting;
- Any case requiring advanced diagnostics: ERG, gonioscopy, or ocular ultrasound.
