For those boarding, please make sure we have a copy of your pet's up-to-date vaccination record before check in, including the actual rabies certificate, rather than an invoice or reminder-type document. If you do not have a copy you can ask your veterinarian to send over a copy for you. You or your veterinarian can send vaccination records including rabies certificate to [email protected] thank you

Here at D and D Kennel we do require pets to have current proof of vaccinations. We do need to keep a copy of your pet's current vaccination record including rabies certificate on file as well as a copy of their license. Please make sure we have your pet's vaccinations record and rabies certificate....

There is an owner and pet information form, an emergency contact form, and boarding contracts (small pet, cat, and dog). There is also a medication form (to fill out if your pet is on any kind of medication, supplement, and or vitamins - if none, the medication form does not need to be filled out).

We also need to have an up-to-date copy of each pet's vaccination record, including rabies certificate (the actual certificate rather than an invoice or reminder type document). If you do not have a copy you can ask your veterinarian to send over a copy for you [email protected]

After forms are submitted you will be contacted regarding reservation confirmation, if we are booked, or with any questions or concerns we may have regarding your form submission.

* Saturdays and Sundays from 8:00 AM to 12:00 PM *

- Working with dogs and cats on a daily basis.

- This is primarily a cleaning position. This position is not to play with animals.
