🦴 Are you ready for the answer to last week’s Radiology Reveal? 🦴Here are the findings and diagnosis for the 8-year-old Manx cat.

A challenging case to crack! 🔍 Radiographs revealed a soft tissue mass-effect near the main airway junction (perihilar area), causing the windpipe (trachea) to elevate and sharply bend. These findings, including an increased distance between the trachea and major blood vessels, is indicative of tracheal bronchial lymphadenopathy. If you can remember from the recent cases in Radiology Reveal, the two main possibilities for this are fungal disease or lymphoma. Unfortunately, confirmed by additional testing, was lymphoma, a type of cancer stemming from the enlarged lymph nodes in the chest. Early diagnostic imaging is key! 🔑

Our patient is a 8-year old, spayed, female Manx cat that has been suffering from coughing and labored breathing for the last 6 months. She has only been able to eat soft food as dry food causes vomiting. Her thoracic films were normal 6 weeks prior.

🦴Happy Halloween from the VitalRADS team! Are you ready for the answer to Monday’s Radiology Reveal? 🦴Here are the findings and diagnosis for the case we shared, the 10-year-old Pitbull suffering from lethargy and anorexia.

The orange circles on the images highlighted the tracheobronchial lymph nodes, which were larger than normal. This can sometimes indicate an issue when their edges aren't clearly defined. We also noticed the trachea, or windpipe, was slightly elevated and had an unusual curve, shown by the red line and blue arrow.

These signs led us to further investigate, and we confirmed the presence of enlarged lymph nodes in the chest area. Unfortunately, our findings pointed to a diagnosis of intermediate to high-grade lymphoma, a type of cancer, confirmed through cytology.

Happy Friday! Here is the findings and diagnosis to Monday’s thoracic images.
The main finding is that the lymph nodes (small, bean-shaped glands that are part of the immune system circled in the orange pumpkin shape) located near where the terminal trachea (windpipe) splits into each side of the lungs are swollen or enlarged. The lymph nodes in this area are called the tracheobronchial lymph nodes. Other lymph nodes located in the thorax are the cranial mediastinal lymph node and sternal lymph node.

This swelling is causing a few things to happen:
• The windpipe (trachea – indicated with an orange line) is being pushed up and is now closer to the thoracic spine.
• The caudal trachea (teal arrow) develops a sharp, sudden bend downward (ventrally).
• There is an increased density on the VD view caudal to the tracheal bifurcation.
o Widening of the caudal lobar bronchi (windpipe indicated by black lines).
o This increased opacity is in the same location as the heart's left upper chamber (Left Atrium).
o The ventral curve of the terminal trachea (teal arrow) is how you tell the difference between an enlarged lymph node versus an enlarged Left Atrium. An enlarged left atrium will not have the curve of the teal arrow.
In short: The tracheobronchial lymph nodes in the chest are enlarged, and this is pushing the main airway (trachea) out of place and creating an abnormal shadow over the heart.
Possible causes for tracheobronchial lymphadenopathy is Lymphoma or Systemic fungal disease

Confirmed Diagnosis: Systemic fungal disease (Blastomycosis) This animal was from the mid-eastern portion of the US, where blastomycosis is endemic.
