FEBRUARY 2025 LOW COST CLINIC REGISTRATION IS OPEN!!!

You must live in Crawford County to take advantage of this deal.

1) Private Message us here on Facebook to receive the registration form.
2) Complete and Submit the form.
3) Pay the fee online or in person on a Saturday.
4) An email will be sent confirming your payment and registration and will give you directions on how to make your appointment.
