A lot of questions coming in about how to acquire your dogs CHIC number. Please see the requirements in the screenshot below.
Note that hips is "ONE of the following". You can have OFA hip scored or submit PenHIP results to the OFA. You do not need to do both. This is not the OFA grading distracted views that you sent to PennHIP, this is you forwarding your PenHIP results to the OFA to be added to their database. The dog must have an existing OFA record of some type. To submit, the owner should email or mail a copy of the PennHIP report to the OFA at: [[email protected]](mailto:[email protected]), or 2300 E Nifong Blvd Columbia, MO 65201. The fee to record these results is $25, and payment must accompany the request. The OFA will record the left and right DIβs only, not the percentile. There is also more information about internationally scored hips being submitted to the OFA in the following link....

Elbows are graded thru the OFA.
Cardiac is an echocardiogram performed by a cardiologist and results are submitted to the OFA.
Remember that all radiographs for OFA must be submitted directly by the attending veterinarian. You can not at any time email the OFA your saved radiographs to be evaluated. Your vet needs to do this for you.
Note that abnormal results must be signed off to be released in the OFA database for you to receive a CHIC number. If you did not sign the release, you will not be awarded a CHIC number.
See example of PennHIP results in the OFA database in the second image.
