Arizona City Dog & Puppy Vaccinations, Microchipping, and Testing Services

This is Not a veterinarian's office, I am Not a Veterinarian, and nor am I claiming to be one. Therefore, All new clients must sign a waiver of acknowledgement and acceptance before any services can be administrator. Rules of services:

* All dogs must be on a leash

* If you have an aggressive dog, your dog must be muzzled at all times.

* Sick dogs or puppies can ONLY receive Parvoviru

s testing.

* All dogs and puppies must be healthy to receive any vaccinations.

* Puppies must be 4 weeks old or older to receive microchipping.

* Dogs must be 6 months old or older to receive a rabies vaccination.

* All services must be paid upfront upon arrival.

* All appointment required a $10 deposit in advance that will go towards final bill. If you are more then 15 minutes late, or is a No Show No Call, your deposit will be forfeited. If you need to reschedule your appointment, you will need to do so within 24 hour of your appointment to avoid forfeiting your deposit.

* No rude or Inappropriate behavior will be tolerated, in person, via text or phone. If you do so, and you have already scheduled an appointment, your appointment will be canceled, you will Not be able to reschedule, and your deposit will be forfeited.
