“Cricoarytenoid Ankylosis: โรคยากของน้องมายา”
ประมาณ 2 ปีก่อน น้องมายาเริ่มมีอาการหายใจลำบากอย่างเห็นได้ชัด คุณหมอสงสัยว่าอาจเกิดจากภาวะกล่องเสียงอุดตัน หรืออัมพาตของกล่องเสียง ซึ่งเป็นปัญหาที่พบได้บ่อยในสุนัขสูงอายุ จึงตัดสินใจวางยาสลบเพื่อตรวจดูความผิดปกติของกล่องเสียง
ผลการตรวจปรากฏว่า กล่องเสียงไม่ได้เป็นอัมพาตอย่างที่คาดไว้ แต่กลับพบว่า น้องมายามีภาวะที่เรียกว่า Cricoarytenoid Ankylosis หรือการที่ข้อต่อกระดูกอ่อนของกล่องเสียงยึดติดและแข็งตัวจนไม่สามารถขยายออกได้ในจังหวะหายใจเข้า ต้องสารภาพกันตรง ๆ เลยว่า ก่อนที่จะเจอจริง ๆ ด้วยตาในเคสน้องมายา คุณหมอก็ไม่เคยได้ยินชื่อโรคนี้มาก่อน 😅 เพราะเป็นโรคที่พบได้น้อยมาก เลยแทบไม่เคยมีใครในวงการสัตวแพทย์เคยพูดถึงหรือเคยสอนกันมาก่อน ต้องกลับไปเปิดตำราค้นคว้ากันเลยทีเดียว ถึงได้รู้ว่ามันมีโรคแบบนี้อยู่จริง ๆ วุ้ย 😲
เมื่อรู้การวินิจฉัยที่แน่ชัด ก็พบว่าการแก้ไข ไม่มีทางเลือกอื่น เพราะน้องคงหายใจไม่ได้ถ้าไม่เปิดช่องอากาศใหม่ น้องมายาจึงได้รับการผ่าตัดเปิดหลอดลมหรือเจาะคอถาวร โดยคุณหมอผ่าตัดเนื้อเยื่ออ่อนผู้คร่ำหวอดด้วยประสบการณ์อันยาวนานของเรา (ตรงนี้พิจารณาจากอายุ 🤣) แล้วก็เหมือนได้ชีวิตใหม่อีกครั้ง น้องมายากลับมาหายใจโล่งสบาย ภายหลังจากการผ่าตัดแทบจะทันที
ถึงแม้ว่าการดูแลช่องคอถาวรจะไม่ใช่เรื่องง่ายเลย เพราะอาจมีภาวะแทรกซ้อนหลายอย่าง โดยเฉพาะในช่วงแรกที่มีความเสี่ยงต่อการอุดตันจากเสมหะ และการติดเชื้อจากช่องทางหายใจใหม่ แต่ด้วยความรักและความทุ่มเทของคุณพ่อคุณแม่ที่ดูแลน้องอย่างใกล้ชิด เวลาผ่านมาแล้วเกือบ 2 ปี มายายังหายใจผ่านท่อได้คล่องอยู่ค่า
ปัจจุบัน น้องมายาอายุ 15 ปีแล้ว มีโรคอื่น ๆ รุมเร้าตามวัย ทั้งโรคหัวใจและปัญหาเกี่ยวกับหมอนรองกระดูก แต่คุณพ่อคุณแม่ก็ยังดูแลน้องด้วยความรักและเอาใจใส่เต็มที่เหมือนเดิม ขอให้น้องมายามีความสุข เป็นกำลังใจดวงน้อยให้พ่อและแม่ไปอีกนาน ๆ นะคะ
About two years ago, Maya started showing signs of significant breathing difficulty. The veterinarian suspected laryngeal obstruction or laryngeal paralysis, both of which are commonly seen in older dogs. So, they decided to perform an examination under general anesthesia to evaluate the larynx directly.
Surprisingly, the larynx wasn’t paralyzed as initially suspected. Instead, it turned out that Maya had a condition called Cricoarytenoid Ankylosis, where the cartilaginous joints of the larynx become fused and stiff, preventing them from opening properly during inhalation. Honestly, before seeing it firsthand in Maya’s case, the vet had never even heard of this condition before—haha! It’s such a rare disease that hardly anyone in the veterinary field ever talks about it or teaches it in class. We literally had to dig through textbooks to confirm that yes, this condition does exist.
Once the diagnosis was clear, we realized there was really no other option—without creating a new airway, Maya simply wouldn’t be able to breathe. So, she underwent a permanent tracheostomy performed by our seasoned soft tissue surgeon (let’s just say their experience level correlates with their age—haha). And just like that, it was as if Maya was given a new life—she could breathe freely almost immediately after the procedure.
Caring for a permanent tracheostomy site hasn’t been easy. There were many potential complications, especially early on—mucus plugs and infections through the new airway, to name a few. But thanks to the boundless love and dedication of her mom and dad, who have cared for her so closely, nearly two years have passed and Maya is still breathing comfortably through her tube.
Today, Maya is 15 years old. Understandably, she’s dealing with some age-related health issues, including heart disease and spinal disc problems. But her parents continue to care for her with the same love and devotion as always. Maya, may you stay happy and continue being a little bundle of joy and comfort for your mom and dad for many more years to come.
