Medical History of Evgeny Mikhalev
Request for Assistance in Accessing the Life-Saving Medication Tarlatamab (Imdelltra)
My name is Anna Mikhaleva, and I am urgently seeking help for my husband, Evgeny Mikhalev, born in 1970.
We have been together for 37 years.
He is the most precious person in my life.
I cannot and do not want to lose him.
For the past year and a half — since June 2024 — we have been fighting relentlessly for his life: continuous treatments, hospitalizations, intensive care, and a daily battle for survival.
⸻
Diagnosis
Evgeny has been diagnosed with small-cell lung cancer (SCLC), confirmed in 2024.
This is one of the most aggressive cancer types and requires uninterrupted medical supervision and therapy.
⸻
Treatment by Leading Specialists
Evgeny is being treated under the supervision of Professor Orlov at the Pavlov First Saint Petersburg State Medical University — one of Russia’s leading oncology centers.
All internationally recognized protocols have been applied, but his cancer has proven refractory to all standard therapies.
⸻
Summary of Treatment History (July 2024 — October 2025)
Evgeny underwent seven lines of anticancer therapy, including combination chemotherapy, targeted treatments, and monotherapy:
1. EP (cisplatin/etoposide)
2. Docetaxel
3. Docetaxel + Bevacizumab
4. Niraparib + Gemcitabine
5. Temozolomide
6. AC regimen
7. Irinotecan (monotherapy)
Conclusion:
Evgeny has exhausted all available standard treatment options.
Despite all efforts, the disease continues to progress.

Many people in the Leonberger community call him “the heart of the breed.”
About the Photo
The picture shows a young girl peacefully walking four of our Leonbergers.
This image reflects everything Evgeny built: harmony, kindness, strength, and a deep connection between humans and animals.
⸻
Current Medical Condition
Evgeny is in critical condition and requires constant medical care:
• severe protein-energy malnutrition
• critically low albumin
• anemia of chronic inflammation
• high LDH and ferritin
• severe edema
• extreme weakness and pain
• recent intensive care
• requires daily IV therapy and nutritional support
Despite all this, Evgeny continues to fight.
He wants to live.
⸻
Why Tarlatamab (Imdelltra) Is His Only Chance
Tarlatamab is the only innovative therapy that has shown significant benefit for patients with small-cell lung cancer who have already undergone multiple lines of treatment.
It offers hope when all standard options have failed.
• FDA approved (2024–2025)
• Demonstrated survival benefits
• In clinical use in the USA
• In Europe available only through Compassionate Use or clinical trials
For Evgeny, this medication represents his only real chance to slow the disease and extend life.
⸻
Treatment Cost and Duration
According to major oncology centers:
• One cycle: 15,000–22,000 USD
• Full course: 4–6 months
• Total cost including diagnostics and logistics:
70,000–120,000 USD
We have raised a portion of the required amount, but it is not enough to begin treatment.
We urgently need help.
⸻
Our Request
We respectfully ask for assistance with:
✔ Access to Tarlatamab (Imdelltra)
Through Compassionate Use / Expanded Access
or financial support to purchase the medication.
✔ Additional support
• arranging treatment at a European clinic
• preparing medical documentation
• international logistics
• partial or full coverage of treatment costs
This medication is Evgeny’s only chance for life.
We are fighting, but we can no longer manage alone.
⸻
Additional Information
After an extremely difficult and exhausting course of anticancer treatments, Evgeny now requires very expensive, continuous supportive therapy to stabilize his condition and restore vital functions. Every day we are doing absolutely everything we can to keep him with us — supporting him medically, nutritionally, and physically.
This is a fight for his life, and we cannot overcome it without help.
