Dr Zahir’s pet clinic

A pregnant hybrid goat was suffering from Dystokia: difficulty in parturition i.e. birthing process.
The owner visited Dr Zahir’s pet clinic as his goat was under the weather while in labor. Dr Zahir diagnosed her with dystokia after performing an x-ray and carried out a cesarian section.
The operation was successful!

Diagnostics:
Diagnosed through Ultrasound and finally confirmed by x-rays.

PROCEDURE(EXPLAINATION):
Following procedure was carried out by Dr Zahir shah:
• Dystocia defined as difficulty in parturition. It is a common condition in small ruminants especially goats. A four-year-old hybrid goat was bought to the clinic with the history of full-term pregnancy and distention of the ventral abdomen. The animal was showing straining intermittently for the past one day without progressing into parturition. Preliminary examinations followed by pervaginal examination identified the need of cesarean section. The goat was sedated using xylazine @0.1mg/kg. Local infiltration analgesia was achieved with 2 % lignocaine hydrochloride at the proposed site of the incision. Left lower abdominal coeliotomy was done. The uterus was carefully detached from the lower abdominal wall and was lifted towards the incision site. Removed the dead fetus from the uterus. The uterus was sutured, followed by suturing of the peritoneum, muscle layers, subcutaneous fascia and skin. Postoperatively animal will be treated with antibiotics(penicillin) for five days. The goat recovered from anesthesia without any complication.

Time:
The operation took about 2hour.

Causes of the dystokia in goat:
In the farmers goat the kids were not in the ideal position of birth.
Common causes of dystokia are following:
-The doe's cervix might not be properly dilated.
-The kid is not in the ideal position for birthing.
-The kid or even just the kid's head is too large for natural birthing by the doe.
-Twins can present for birth at the same time.
-Stress due to a disturbance or fear during early labor can even interfere with the normal birthing process.
Additionally, health problems that put excessive stress on a goat going into labor can cause delivery challenges. These conditions include:
-Toxemia
-Ketosis
-Hypocalcemia (milk fever)
-Mastitis
-Congested udder
-Pr*****ed uterus

Feedback:
In such cases in order to save the life of the pregnant animal surgery is the best option.

Zoy a dog met with a serious accident and her femur fractured. She was all black and blue but the owner found it out after a few days; when she was observed laying down most of the time and unable to stand on the fractured limb. Zoy visited Dr Zahir’s Pet clinic where she was diagnosed with the fractured femur. The operation was successful!

Following procedure was carried out by the doctor:

Symptoms/Diagnostics:
• Physical examination was carried, finding femoral fractures: Usually non-weight bearing and having a varying degree of limb swelling are signs of fracture; pain and crepitus can often be elicited with limb manipulation.
• The animals reluctance to move the limb due to serious pain conditions.
• Both cranio-caudal and lateral radiographs of the femur are necessary to assess the extend of bone and soft tissue injuries.
• The x-ray confirmed the fractures in the femur.

Procedure:
The following surgical procedure and treatment was carried out as preferred:
• Surgical treatment:
IM pins or bone plates were used to repair femoral fractures. Preoperative management as these fractures result from trauma, all affected animals should be examined fir concurrent injury.
• Anesthesia:
An epidural anesthetic may make fracture reduction easier along with general anesthesia. Which was observed by the vet.
• Positioning:
The patient was positioned in lateral recumbency.
• Surgical technique/ surgical approach to the femoral diaphysis fracture:
Incise the superficial leaf of the fascia lata along the cranial border of the biceps femoris muscle. Retract the biceps femoris caudally to expose the vastus lateralis as it inserts at the caudal lateral border of the femur. Reflect the vastus lateralis from the surface of the femur to expose the femoral diaphysis.
• Application of intramedullary pins:
An IM pin can be used to stabilize femoral diaphyseal fractures, providing excellent resistance to bending but not resisting rotational forces or axial loading. Generally, an IM pin should equal 70% to 80% of the diameter of marrow cavity. The pin must be for proper seated within the femoral condyles.
To retrograde a pin, expose the fracture and insert the pin into the medullary canal of the proximal segment. Secure the proximal bone segment with a bone holding the forceps and drive the pin proximally, forcing the shaft of the pin against the caudomedial cortex to help ensure that the pin exists laterally in the fossa. Hold the limb in a hip extended and adducted position while the pin exists the trochanteric fossa to prevent pe*******on of the sciatic nerve. Pulled the pin prox-imally until the point is within the canal. Reduce the fracture, and drive the pin distally.
• Postoperative care and assessment:
Activity should be restricted to leash walking and physical rehabilitation until the fracture has healed. Physical rehabilitation encourages controlled limb use and optimal limb function after fracture healing. Radiographs should be taken and evaluated after every 6 weeks.
• Complications (maybe):
Complications include delay union, non-union, malunion, osteomyelitis, pin tract infection, and fixation failure. Sciatic nerve injury occurs due to improperly placed IM pins. Premature loosening and migration of IM pins.

Prognosis:
Prognosis is generally excellent if proper fracture management procedures are followed.

Time:
The operation took about 3hour.

Feedback:
In such cases the owner should take good care of the pet and follow all procedures with full zeal in order to get speedy and accurate recovery of the pet.

High velocity fracture trauma resulting in disruption of the continuity of the diaphyseal cortical bone.

Chemi the bitch was suffering from Uterine tumor (Leiomyomas and Pyometra). The tumor had taken a deadly shape and it was no more in its initial shape.
The owner of Chemi a busy bank manager as observed the deteriorating condition of his beloved pet, searched out the globalized internet in order to get the best veterinary surgeon in town. Hence, he found out about Dr Zahir’s clinic and visited as he
got the cordial appointment.

Following procedure was carried out by the doctor:

SYMPTOMS:
Initially Chemi was lethargic and her movement was abnormal. Then the swollen abdomen was observed. Later on, droppage of pus was also observed. The urine had pus as well. Chemi was in pain. Moreover, her appetite also reduced.

DIAGNOSTICS:
The tumor was quite visible through observation of symptoms; in order to confirm it an ultrasound and an x-ray was carried out.
The x-ray confirmed the expansion of tumor.

DISEASE:
UTERINE TUMORS:
Leiomyomas is the most common cancer, found in the smooth muscles of the walls of uterus.
While pyometra which is pus in the uterus. It occurs in about 25% of not spayed female dogs. In essence, pyometra is a uterine infection aiding in the development of uterus cancer.

PROCEDURE:
The following procedure and treatment was carried out as preferred:
Xylazine 0.2mg/kg
Ketamin 8mg/kg
Atropine 0.02mg/kg
· Chemi was give anesthesia as required according to her condition and body weight.
· Intravenous fluids were passed before and after the surgery to stabilize chemi.
· Surgery was carried out; removing the uterus and ovaries performing an ovariohysterectomy (spay).
· Antibiotics and intravenous fluids were carried on.

TIME:
The operation took about 1hour.

FEEDBACK:
Chemi arrived when she was critically ill and the cancer had already spread its destruction in Chemi’s reproductive organs, effecting the overall working, and health of the body.
According to veterinary surgeon, Dr Zahir Shah, dogs and bi***es diagnosed at early stage of the disease are very good surgical candidates. The surgery is more complicated than a routine spaying in such a case. Moreover, most dogs are diagnosed with pyometra when they are quite ill, resulting into a more complicated surgical procedure and longer period of hospitalization. Intravenous fluids are required to stabilize the dog before and after surgery. Furthermore, antibiotics are usually given for two weeks after surgery.

CAUSES OF THE DISEASE: UNKNOW

HEALING PROCESS:
Generally, it takes two weeks for the major healing. While the dog is under observation for a month after the procedure. Female procedures are more complicated than males.
Incase of Chemi, she was at a very critical stage. She did not survive.

A dog was suffering from a benign tumor. The owner visited doctor Zahir’s clinic after finding out about the clinic from social media. Following procedure was carried out by the doctor;
Diagnostics:
According to the doctor, a common procedure to diagnose this tumor is Fine Needle Aspiration (FNS).
FNA involves, taking a small needle with a syringe and suctioning a sample of cells directly from the tumor and placing them on a microscope slide. A veterinary then examines the slide under a microscope.

Disease:
Name: benign tumor
Description: clinical sign is palpable masses underneath the skin of the abdomen. They may be next to or within the ni**le and flow along the mammary chain. This size of the masses and their appearance may vary but they are usually firm and nodular.

Procedure:
The dog was given adequate amount of anesthesia. Further the tumor was removed by the tooth and nail efforts of the doctor. The dog was in consciousness soon after the treatment. A follow up medication was prescribed accordingly. The surgery was successful. The clients left happily.

Feedback:
The bitch has recovered and is in the pink now.

Causes of the disease are unknown.

Healing process :
it takes about 7 to 10days.

Doctor Zahir shah, a veterinary surgeon, practicing and living in Peshawar. Saving and treating animals is not what he does, it is who he is. He did his DVM-doctor for veterinary medicines- from university of Sindh. Furthermore, having the spark of getting more educated and specialized in his field he further did his MS in surgery and PhD from China in surgery and obstetrics. Further, he joined the university of agriculture Peshawar as a professor and is serving since the last 20years. Currently he is an associate professor in the university, teaching veterinary students. He also has a spacious clinical set up in Peshawar where he practices and treats animals from 4pm onwards. He is specialized in clinical veterinary medicine and surgery while his expertise are in surgical interventions.
Doctor Zahir’s stay in China for his PHD program proved to bring back a well seasoned veterinary surgeon in Peshawar. Who not only came back to Pakistan with his sharp mind full of knowledge but also bought in practice his healing hands -gifted- saving and treating many suffering animals across Pakistan.

I had operated a cat with tumor in her ear which was declined by many vets due to the sensitivity of ear while doctor Zahir successfully operated her. I also got my various pets neutered. Currently we are in the process of getting a Labrador pup operative for his broken bone. Hopefully the Labrador will return home in the pink soon.
