A patient presents with severe azotemia, vomiting, and elevated phosphorus. The kidney values tell us that renal function is impaired—but they don't tell us how long the disease has been present.

Differentiating Acute Kidney Injury (AKI) from Chronic Kidney Disease (CKD) requires us to look beyond the laboratory results and focus on the patient in front of us.

🔹 AKI patients often maintain normal body condition and muscle mass, may have large painful kidneys, and are less likely to be anemic.

🔹 CKD patients commonly show weight loss, muscle wasting, anemia, and small irregular kidneys due to a chronic disease process.

Of course, this is a simplified comparison. Not all patients will fit neatly into either category, and acute-on-chronic kidney disease is a common clinical presentation.
