Perineal Urethrostomy in Cats: A Lifesaving Surgery for Urinary Blockage

Urethrostomy in cats, particularly perineal urethrostomy (PU), is a surgical procedure performed to create a new permanent opening in the urethra to allow easier urination. This is most commonly done in male cats who suffer from recurrent urinary obstructions due to urethral plugs, crystals, stones, scarring, or trauma (especially when medical management fails). The surgery involves removing the narrow portion of the pe**le urethra and suturing the wider pelvic urethra directly to the skin, typically in the perineal area (between the a**s and sc***um as shown in the image).

Before the procedure, stabilization of the patient is critical, including fluid therapy and correction of any electrolyte imbalances. Diagnostic tests such as urinalysis, bloodwork, and imaging are also done to assess the underlying cause.

After surgery, strict post-operative care is required, including the use of an e-collar to prevent licking, switching to paper pellet litter to avoid infection, pain management, and monitoring for bleeding or swelling.

Although complications such as bleeding, infection, or surgical site narrowing (stricture) can occur, most cats recover well and are able to urinate comfortably afterward.

With proper aftercare, the prognosis is generally good, and the procedure significantly improves the cat’s quality of life by preventing life-threatening urinary blockage.

A female dog was diagnosed via X-ray with bladder stones and a mummified fetus, both discovered during evaluation for abdominal discomfort and abnormal discharge.

She underwent successful surgical removal of the stones and fetus under gas anesthesia and made a full recovery.

The fetal mummification was likely due to blood parasitism (Ehrlichiosis), which can cause maternal anemia and poor placental circulation, leading to fetal death. To prevent fetal mummification, it’s essential to test and treat for tick-borne diseases before breeding, maintain parasite control, and ensure overall reproductive health. This case highlights the importance of pre-breeding health checks and timely veterinary care to prevent life-threatening complications.

Bladder stones commonly form due to urinary tract infections, mineral imbalance, or concentrated urine. Prevention includes feeding a balanced, vet-recommended diet, ensuring constant access to clean water, maintaining proper urinary pH, prompt treatment of UTIs, and regular urinalysis or imaging for predisposed breeds.

The 1st one is a female dog with a mammary tumor was found during surgery to also have tumors in both ovaries, prompting a combined mammilectomy and ovariohysterectomy.

The second dog is with a uterine tumor that underwent an ovariohysterectomy.

Both procedures were performed under gas anesthesia, which offers significant advantages such as rapid induction and recovery, precise control over the depth of anesthesia, and a reduced metabolic burden since the anesthetic agents are primarily exhaled. These benefits help minimize surgical risks and enhance overall patient safety.

Furthermore, these cases highlight the importance of spaying as a preventive measure; early spaying not only drastically reduces the risk of developing mammary, ovarian, and uterine tumors but also prevents life-threatening conditions like pyometra, ultimately contributing to a longer, healthier life for female dogs.

Here’s why this phrase is actually a green flag, with examples from real-life clinic situations:

We want to keep you informed, even when things don’t go as expected.

Example:
“We weren’t able to get a clear ultrasound image of your pet’s abdomen because they were too anxious to stay still during the procedure.”
🐾 Why this is a green flag: Instead of giving you incomplete or inaccurate results, your vet is upfront about the challenge and may suggest other alternative diagnostic tools to get accurate results.

A client brought in their dog with a bone lodged in the esophagus. We examined the dog and determined that trying to remove the bone ourselves could cause serious damage to the esophagus, so we suggested referring the pet to a specialist. The client later visited another vet, who successfully removed the bone. However, the client expressed frustration, saying, “You didn’t know what you were doing.”
🐾 Why this is a green flag: Our decision to refer was based on the potential risks of trying to perform the procedure ourselves. While the other vet was able to successfully remove the bone, our priority was to ensure the dog’s safety by making the right decision, even if that meant referring them for specialized care.

As groomers, we always aim to provide a great grooming experience, but sometimes a certain style might not be suitable for your pet’s condition.

Example:
