Congratulations to Willow as she completed her extended laser therapy sessions today. We’ve been doing at least one session per week for the past 7-8 weeks.

She was referred by a colleague for bilateral femoral head and neck ostectomy done 2 months apart.

Riight leg recovered much earlier than expected, while the left side took a lot longer time to recover so we had to extend her laser therapy sessions.

Owner was very compliant as they still come from San Juan , Metro Manila and never missed a session.

I’m happy now as she is already walking really fine and I think I won’t be seeing her for a long time.

This goes to show that not all FHO surgeries have the same predictable recovery time. Span usually takes 6-8 weeks of rehab and physiotherapy.

There’s the dog being nosy about his histopath results.

This dog had chronic anemia and was found out to have a massive spleen. It was a splenic tumor just waiting to rupture.

I removed his spleen a few weeks ago then sent a sample of the spleen to histopath.

Now the results are back and it was diagnosed as splenic hemangiosarcoma— basically a form of cancer that affects the lining of the blood vessels.

This is an aggressive form of cancer in dogs, so I had to call the owner back to discuss and plan for chemotherapy.

Chemotherapy in dogs is not much the same as chemotherapy in humans. They don’t usually go bald— although there are other side effects to watch out for, like vomiting, diarrhea, lethargy, anemia, etc.

Chemotherapy does not provide 100% cure in dogs — it is more of a way to improve their quality of life.

Some owners will go for chemotherapy, some owners will not, either because the cancer is already way advanced, or because of the time and financial commitment that comes with chemo.

As a veterinary professional, I always find myself in a position to deliver both good news and bad news.

Surgery was done successfully, but it was just one part of the story. The next part is going through treatment and hopefully making it well onwards.

Always Have a Plan B

I’m supposed to do a unilateral perineal hernia repair with internal obturator muscle transposition technique but on this dog his internal obturator muscle is just so small and poorly developed.

So we ended up using the traditional repair technique with higher chance of recurrence. (This was explained to the owner)

There is a described technique in the books called a semitendinosus muscle transposition technique to repair a perineal hernia and this will be considered if there is recurrence of the hernia in the future.

Also, the original plan was for him to stay at the clinic overnight at the clinic to monitor and for pain management purposes but as soon as he woke up, he started barking and was anxious at the cage.

You really don’t want a barking dog overnight with this kind of surgery bec of the increased abdominal pressure so we ended up calling the owner to pick him up and to stay at home instead.

This morning I asked them to drop by at the clinic just to make sure he’s doing okay. He was doing okay and seems to be fine and painless and with good appetite after the surgery.

Take home message is that to always have a plan B.

Veterinary medicine is not an exact sicience and sometimes the planned outcome will not go as expected.

Over the years I had to learn to be flexible and accept that my profession is a combination of both art and science - about 50% of cases that we deal with will be dealt with thru years of experience.

There is no exact cookie cutter solution that fits all pets and pet owners.

For example…

Unique anatomical differences.
Pet has unpredictable response to treatment.
Owner has a busy schedule.
Owner has financial concerns.
Pet is extremely anxious and difficult to handle.
Dog or cat prefers a tablet than syrup and vice versa.
Owner lives quite far from the clinic.

Etc etc.

Books will tell you to do this or do that. But experience will teach you to have backup plans.

As I talk to clients on the consultation room, I lay out what is the treatment plan, but also in my mind I am also thinking of a backup plan.

Helping this Aspin with a large mass dangling from his right rump.

We made sure to get good margins on this one as it will not go into histopath.

The good thing is that it is not invasive in tissue growth and the base that is attached to the thigh muscles is much smaller than the size of the mass.

After the surgery, he must have felt instantly lighter because the mass is 600 grams.

We get many surgical cases of lumps and bumps and about half of these cases will be cured with surgery alone.

The other half will require additional chemotherapy to complete the treatment process.

The key is to have them checked as early as possible because sometimes when tumor growth becomes expansive, surgery will be more difficult.

We like to post our surgical cases but we really offer a lot more than surgeries. We offer consultations, preventive healthcare, in house laboratory, confinement, etc.

Subtotal Colectomy As Surgical Treatment for Chronic Constipation in Cats

When the large intestine is chronically constipated for at least 6 months, the patient will develop megacolon and the ability of the large intestine to contract and expel stool is diminished or totally gone.

Patients with megacolon will also NOT respond anymore to medical treatment to relieve the constipation.

When this happens, we will have to resort to a surgical procedure called subtotal colectomy, which involves removing about 90% of the colon.

This procedure is not without the risk as it involves cutting off a large segment of the instestine then suturing the re**um and small intestine together.

Such is the case of Abel, a 6 year old male Puspin that we have been treating for chronic constipation for atleast 3x already.

Eventually we opted to do the surgery last month because he was not responding anymore to medical treatment.

The surgery went well and now is able to defecate on his own without meds and already free of chronic constipation.

The only downside of the procedure is that they will not produce hard, formed stools, but they will produce a softer stool with no definite form.

But hey, if I were a chronically constipated cat, I would rather have the latter than not being able to p**p at all.🤗

When Letting Go is the Kindest Thing

Its not uncommon for me to be performing euthanasia procedure (put to sleep) in a day’s work at the clinic.

But today hits different because I had to perform two — one is a 14 year old Aspin with multiple inoperable tumors, and a 12-13 year old Puspin with chronic kidney disease who’s been in and out of confinement 3 times already and now both patients are not doing well and not eating anymore.

The primary role of a small animal veterinarian is to provide primary care to pets such that disease condition is treated, with no pain and suffering in between.

When the body reaches its limit, such that we cannot do so much anymore as vets, it is also our role to advocate for them, who cannot tell their owners that they are suffering and convince the owners the kindest thing to do is to let them go.

Many many years ago I took an oath to protect animals from pain and suffering, and up until today I am bounded by this oath.

For me its not just my job, but giving them a dignified exit when its time to go…

And even though I will be seeing another patient after the procedure, rest assured my heart shares the loss of a family member, and my sympathy is with the pet parent who lost a beloved one..

Dental Extractions, Is It Worth It?

Perhaps one of the most satisfying surgical procedures that we do, when it comes to results, are dental extractions.

After extractions, owners report that their pets have been eating way better, and some even say their dogs or cats got a new lease of life.

I remember one client saying “parang naging bata siya ulit doc, naglalaro na ulit at mas masayahin na. Nakikipag agawan na din ng pagkain. Dati hindi.”

When you hear reports like this you feel really good as a vet because you know you did well for the patient.

Dental extractions offer a lot of health benefits to pets. We remove these rotten teeth to alleviate pain and discomfort, and to eliminate the constant source of bacteria being transported into the system through the highly vascular oral cavity.

Some owners may be apprehensive, and I get that. Their furbaby will undergo general anesthesia and potentially receive multiple extractions.

But as I always say, performing general anesthetics without physical exam and pre anesthetic blood work is like crossing the road without looking to your right or to your left - that’s what makes it dangerous.

Again, even an excellent physical exam and excellent blood work is not 100% safe guarantee.

There is a 1% chance your pet may react badly to the meds given, including the anesthetics and pain meds.

So is it worth it taking all the risk and going ahead with the dental?

Absolutely yes.

Benefits will outweigh the risks if you think about the quality of life after extracting all these rotten teeth.

If these teeth don’t get extracted it will cause them to have secondary problems related to periodontal disease, like heart disease, kidney problems, and advancing bone infections in the oral cavitiy., not mentioning the constant source of pain, inflammation, and discomfort.

Common and expected complications related to the extractions include pain, bleeding, and iatrogenic soft and hard tissue injury like mandibular fracture in extreme cases.

The best way to get around these complications is really being thorough with pre anesthetic physical exam and blood testing, and the proedure being carried out by an experienced vet, along with basic anesthetic monitoring practices.

Hi, welcome to our page.

We started serving pet owners in Marikina City in January 2016. Unsure if the business will succeed or not, it was all pure faith and hard work to have survived the first two years of business in a city where various other veterinary practices have already been established and well known for many years.

It was all agony in the first few months waiting for clients to turn up. But with every single client that came in, we made it a point to serve them with all our heart. Slowly, our client base started to grow, and some clients started referring us to their families and friends, and some clients find us through search engines and social media.

Today, we serve pet owners not only from Marikina but we also have some clients coming all the way from Quezon City, San Mateo, Cainta, Antipolo, and Pasig City.
