BJØRN, a 4-year-old Staffordshire Bull Terrier, named after two brilliant legends: tennis genius Bjørn Borg and guitar virtuoso Bjørn Berge. Unfortunately, BJØRN suffered a fracture of the first phalanx of his fourth toe. 🦴

When fractures like this do not involve the joint and are minimally displaced, they can often be treated conservatively. However, when they are intra-articular and significantly displaced, anatomical reduction is essential to minimize the progression of osteoarthritis and "ensure" a pain-free future. 💡

In life, having great mentors is invaluable. My key teachers for distal extremities have been the legendary Jon Dee and my friend Alessandro Piras (too young to be legendary just yet 😉). I’m forever grateful for the lessons they’ve taught me: precision in distal extremities matters even more. 🙏

📸 (Throwback to my days as a participant in the AO/SCIVAC distal extremity course, before becoming an instructor for many years. While I may disagree with AO's policies, I strongly recommend young colleagues attend their basic courses to grasp the fundamentals of osteosynthesis.)

In BJØRN’s case, with the help of my incredible staff, we managed to place two 1.5mm diameter compression screws just 1mm from the joint. To support the osteosynthesis, a Robert Jones bandage will be applied for a few weeks. 🤞 Fingers crossed, and we hope the postoperative instructions are followed diligently.

Every surgery must begin with detailed planning just like every chess game ideally begins with the perfect opening move. Surgery is all about attention to detail. At Clinica Veterinaria Milano Sud, we try to analyze every detail to aim for the best outcomes for our furry loved patients.

The PGR was applied because the patella and trochlea were affected by cartilage erosion as arthroscopically documented. The double locking plate minimizes the risk of implant failure, especially since the osteotomy is in addition (opening wedge) in a very active heavy (47kg) Labrador Retriever. The PGR was applied following the technique of Luca Omodeo, modified by myself (using the surgical burr) to countersink the PGR baseplate into the metaphyseal bone in order to minimize the risk of prosthesis–patella impingement during knee flexion. The target of aLDFA of 95° was achieved as planned. 🤞✌️

Before the surgery, we ensured through arthroscopy that there were no injuries to the cranial and caudal cruciate ligaments, no lesions to the medial and lateral menisci, and documented cartilage erosion on the ventral surface of the patella and the medial ridge of the femoral trochlea. Planning was done using vPOP PRO, and the trochlear implant is an original PGR by KYON plates are original Fixin plates by Intrauma VET.

🐾Vito the Bionic Cat 🐾

Six years and three months ago, destiny brought Vito into our lives. His journey was one of courage and transformation as we helped him regain his mobility and dignity by fitting him with two amputation prostheses. 🦾🐱

Recently, Vito has shown a slight limp, particularly stiffness in his right hind leg. This serves as a gentle reminder of the importance of regular follow-ups for pets with prostheses. To ensure Vito’s comfort and mobility remain uncompromised, he’ll be visiting us for a check-up soon.

These periodic assessments—ideally every six months—allow us to:

✅ Evaluate the condition of the prostheses.

✅ Replace or resole the "shoes" when they show wear.

Ieri mattina, un grave incidente ha coinvolto un’auto e un furgone con a bordo una decina di cani, lasciando una donna di 45 anni in condizioni critiche. Tutti i nostri pensieri vanno a lei, con l’augurio che possa riprendersi presto. 💙
Tra gli animali coinvolti, Kora, una dolcissima Rhodesian Ridgeback, è stata operata oggi presso la Clinica Veterinaria Milano Sud per una br**ta frattura. L’intervento chirurgico è durato due ore e, ed è andato molto bene e ora Kora sta iniziando il suo percorso di recupero. 🏥🐶
Cogliamo l’occasione per ricordare a tutti l’importanza della prudenza alla guida, perché un attimo di distrazione può avere conseguenze molto serie.
Un abbraccio a chi è stato coinvolto, e tanta forza a chi sta affrontando la convalescenza. 💪✨

Yesterday morning, a serious accident occurred involving a car and a van carrying around ten dogs. Sadly, a 45-year-old woman was critically injured. Our thoughts are with her, and we sincerely hope for a swift and full recovery. 💙
Among the dogs involved, Kora, a sweet Rhodesian Ridgeback, underwent surgery today at the Clinica Veterinaria Milano Sud. The procedure was successful, and she is now starting her recovery journey. 🏥🐶

📢 New Publication on TTTT2! 📢

We are proud to announce the publication of our study in Veterinary and Comparative Orthopaedics and Traumatology (VCOT), which biomechanically compares the Tibial Tuberosity Transposition Tool 2 (TTTT2) with traditional fixation techniques for tibial tuberosity transposition in dogs with medial patellar luxation (MPL).

✅ TTTT2 demonstrated superior strength compared to traditional techniques although the difference was not statistically significant.

✅ This study confirms that TTTT2 is a biomechanically valid technique and an excellent alternative to traditional methods.

Dodo, a 6-year-old West Highland White Terrier, first came to us five years ago with a Grade 1 Medial Patellar Luxation (MPL). At that time, a simple surgical procedure—a mild tibial tuberosity transposition combined with a possible sulcoplasty—could have solved the issue permanently. However, his owners opted against surgery.

🚨 Today, after five years, Dodo presented with Grade 3 lameness, a partially positive drawer test and tibial compression test, and Grade 2 MPL in his left stifle.

🔬 Diagnosis confirmed in arthroscopy:
✔️ Partial cranial cruciate ligament (CCL) tear (early-stage rupture)
✔️ Full-thickness erosion of the ventral patellar surface
✔️ Erosion of the proximal medial trochlear ridge of the femur

🦴 Surgical treatment performed:
✅ TPLO (Tibial Plateau Leveling Osteotomy)
✅ Trochlear prosthesis

❗ Why early intervention matters:
Dodo’s continuous untreated patellar luxation over the years led to persistent tibial internal rotation, putting excessive stress on the cranial cruciate ligament, eventually causing its rupture. What could have been a simple procedure three years ago turned into a much more invasive surgery today.

⚠️ Early diagnosis is crucial, but so is timely intervention. Addressing orthopedic issues before they worsen can prevent serious joint damage and the need for complex surgical solutions.

🎉 Celebrating a Milestone in Veterinary Orthopedics! 🎉
Yesterday, Professor Bruno Minto from the University of Jaboticabal, São Paulo, Brazil, along with co-authors , , , and , presented the TTTT (Tibial Tuberosity Transposition Tool) technique at the VOS (Veterinary Orthopedic Society)—one of the most prestigious orthopedic congresses in the world!
This instrument is the predecessor of the TTTT2, which is widely used today, and we want to express our deep gratitude on behalf of myself and the scientific community for their dedication and contribution to the development of this technique.
📊 Results from their study:
✅ No cases of patellar reluxation were observed.
✅ Complication rate: 13.33%—one minor complication (fracture of the osteotomized fragment during translation) and one major complication (implant migration).
✅ At 60 days postoperatively, no patients presented with lameness.
✅ Only one dog required surgical revision due to implant migration.
✅ Bone consolidation outcomes:
• At 30 days: Excellent in 12/15 (80%), good in 1/15 (6.66%), and fair in 2/15 (13.33%).
• At 60 days: Consolidation was excellent in all dogs.

This is the story of Sofì, an 8 kg female poodle, who was diagnosed with a Grade 1 patellar luxation in her left stifle when she was young. Since it wasn’t treated at the time, now at 6 years old, her stifle is in terrible condition.

She has developed severe erosion of the ventral surface of the patella, exposing the subchondral bone. Worse yet, a large osteophyte formed over time, which eventually fractured due to the lever arm effect of the quadriceps extensor mechanism, causing a fracture of the patellar apex.

Sofì did not have any angular deformities, but she had trochlear dysmorphia, with a flattened trochlear groove. For this reason, no corrective osteotomies were performed.

Dogs diagnosed with Grade 1 patellar luxation at 6 months or 1 year of age are destined to worsen over time. It is neither wise, ethical, nor moral to leave them untreated, as the untreated defect will inevitably lead to severe clinical problems.

Now, six years later, Sofì has not only worn away the cartilage of the patella and the medial trochlear ridge, but she has also begun to develop a cranial cruciate ligament rupture.

Today, we performed an arthroscopy, a trochlear prosthesis, and a TPLO, addressing a far more complex issue with far more complex surgery—something that could have been prevented with simpler procedures six years ago.

Prevention and early intervention make all the difference.

Questa è la storia di Sofì, una barboncina di 8 kg, a cui da giovane era stata diagnosticata una lussazione di rotula di grado 1 al ginocchio sinistro. Non essendo stata trattata all’epoca, oggi, a 6 anni, il suo ginocchio è in condizioni disastrose.

Ha sviluppato una grave erosione della superficie ventrale della patella, con esposizione dell’osso subcondrale. Ancora peggio, con il tempo si è formato un grosso osteofita, che alla fine si è fratturato a causa del braccio di leva del meccanismo estensorio del muscolo quadricipite femorale, portando a una frattura dell’apice della patella.

Sofì non presentava deformità angolari, ma aveva una dismorfia della troclea, con un solco trocleare appiattito. Per questo motivo, non sono state eseguite osteotomie correttive.

I cani con lussazione di rotula di grado 1 diagnosticata a 6 mesi o 1 anno di età sono destinati a peggiorare nel tempo. Non è saggio, etico o morale lasciarli senza trattamento, perché il difetto non corretto porterà inevitabilmente a gravi problemi clinici.

Ora, dopo sei anni, Sofì non solo ha consumato la cartilagine della patella e il labbro mediale della troclea, ma ha anche iniziato a sviluppare una rottura del legamento crociato craniale.

Oggi abbiamo eseguito un’artroscopia, una protesi di troclea e una TPLO, affrontando un problema molto più complesso con una chirurgia molto più complessa—qualcosa che si sarebbe potuto prevenire con interventi minori sei anni fa.

La prevenzione e il trattamento precoce fanno la differenza.

Meet Birkin, a 7.5 kg female Cavalier King Charles Spaniel, born in 2017 and now 8 years old. She arrived at our clinic with a severe Grade 4 lameness caused by a traumatic hip luxation on a dysplastic base. Thanks to a cemented hip prosthesis, her recovery has been nothing short of amazing!

Now, just two weeks after surgery, her gait is almost indistinguishable from that of a normal dog. It’s incredible how today’s technology and surgical advancements can deliver such immediate results when everything goes as planned!

Ora, a sole due settimane dall’intervento, la sua deambulazione è quasi indistinguibile da quella di un cane normale. È incredibile come la tecnologia e la chirurgia moderne possano offrire risultati così immediati quando tutto va per il verso giusto!

🌟 Arthroscopy and TPLO for Milou 🌟

I’d like to share the case of Milou, a beautiful and sweet 4-year-old mixed-breed female dog, who today underwent a combined surgical approach at CVMS. 🐾

Milou presented with lameness, and after a clinical and arthroscopic evaluation, we diagnosed her with a partial cranial cruciate ligament rupture and, as expected, non-septic arthrosynovitis. Thankfully, all other structures within the stifle, including the menisci, were within normal limits.

Before proceeding with a TPLO (Tibial Plateau Leveling Osteotomy) to stabilize the stifle, we performed an arthroscopy. It’s incredible how advanced 4K technology, allows us to visualize such fine details. During the procedure, we could even observe the tree-like branching of the microvessels associated with the inflammatory process on the long digital extensor tendon! This technology has revolutionized our ability to make precise diagnoses and optimize treatment plans.

Milou was a brave and cooperative patient, and we are confident that this integrated approach will help her return to a pain-free, active life soon. 🎾
