Standard TPLO works. But there's a step worth rethinking.

In conventional TPLO, achieving rotation typically involves placing a pin in the proximal tibial segment. It works — but it commits the surgeon early, and adjustments during rotation are limited.

X-TPLO takes a different path: rotation is achieved progressively, in a controlled and stable way, without pins in the proximal segment. The result is a procedure the surgeon can manage with greater consistency, with adjustments possible during the rotation itself.

It's not a replacement for TPLO. It's an evolution for surgeons who already master the standard technique and want more control over the rotational step.

In juvenile hip dysplasia, every month matters.

The biomechanics are simple: if the force containing the femoral head within the acetabulum is insufficient, joint development goes wrong — and the window to intervene closes fast.

RATO (Rovesti Abduction Trochanteric Osteotomy) addresses the problem at its biomechanical root. By lengthening the trochanteric lever arm through controlled, progressive abduction after osteotomy, we increase the containing force on the femoral head — working with the dog's growth, not against it.

Other procedures have approached this concept. RATO introduces original instrumentation and a refined technique that make the procedure consistent and reproducible.

Internal or external osteosynthesis? It's the wrong question.

Internal fixation gives you stability — but exposure means biological damage, and small plates risk fatigue failure or demineralization in small dogs.

External fixation preserves biology — but closed reduction demands experience (often fluoroscopy), and refracture after fixator removal is a real concern.

So why treat them as alternatives?

Hybrid Internal/External Fixation (HIEF) uses both: a small plate to stabilize fragments, an external fixator to provide the leverage arm. Each compensates for the other's limits. Minimally invasive access. Immediate loading. Safe fixator removal once callus has formed.

Three surgical philosophies. One workshop. Slovakia, May 15–17.

This week we'll be in Slovakia for a hands-on workshop covering three procedures we've developed and refined for advanced orthopedic problems in dogs:

→ HIEF (May 15) — Hybrid Internal/External Fixation. The case for stopping the "internal vs external" debate and combining the strengths of both for fracture treatment.

→ RATO (May 16) — Rovesti Abduction Trochanteric Osteotomy. A biomechanical approach to early juvenile hip dysplasia, working with the dog's growth through controlled trochanteric abduction.

→ X-TPLO (May 17) — A controlled, progressive, stable rotation for cranial cruciate ligament treatment, without pins in the proximal tibial segment.

Three techniques, one shared logic: rethinking how we approach the problem before choosing what to use.

Over the next few days we'll be sharing the clinical reasoning behind each one.
