THE IMPORTANCE OF INTUITION IN CANCER SURGERY

Despite all the new developments in chemotherapy, radiation and other modalities, surgery is still the most effective option for managing cancer and is most likely to effect a cure, especially for localized disease.
Very few veterinarians have specific training in tumor surgery. With the exception of a small number of specialty oncology operations, most surgical procedures performed by surgical oncologists are similar to those done by surgeons without oncology training.
What sets a good cancer surgeon apart from other surgeons? It is not just his (or her) knowledge of how to perform a specific surgery but also an intuitive awareness of when and how to perform that surgery and how aggressive to be with the surgical margins keeping adjacent structures and organs and their functions in mind. It makes the difference between success and failure, and in the case of cancer, the chance for a cure.
Good cancer surgeons need to understand cancer, its presentation, its effects and it recurrence. They must possess both a cognitive and an intuitive awareness of what drives tumor growth and dissemination and translate that awareness into practical action during surgery. Most importantly, they need a personal philosophy driven by empathy for the patient and a desire to do what's best for them, even if it sometimes means foregoing surgery.
I recall with great clarity the day that I had to decide on a surgeon to operate on my ovarian mass which had already disseminated into my abdomen, and the recommended surgeons I interviewed. It was this very intuitive quality that impelled me to put myself in the hands of the surgeon I chose, Dr Amish Dalal, and I know this has played a very large role in my being alive and cancer free today, 9 years later. I am so grateful to him for actually giving me the chance to live and follow in his footsteps for my patients. Surgery has always been a passion for me, and has driven me into training and honing my expertise with various surgical disciplines, but being a cancer survivor myself has brought a whole new dimension into my surgical journey. It has hard wired the ability to think like a cancer surgeon and make split second surgical decisions that make a very significant difference to the final outcome in dealing with cancer.
Going through the cancer experience is not something I would wish on anyone, but a very positive outcome for me has been the intuitive understanding and empathy it has generated.
