This DCM + Pulmonary Hypertension case highlighted the importance of combining echocardiography with biomarkers like NT-proBNP for better assessment of myocardial stress, chamber remodeling, prognosis, and treatment planning.

Understanding the difference between volume overload, systolic dysfunction, pulmonary vascular changes, and secondary cardiac remodeling is what truly changes outcomes.

Cardiology teaching moment 😄🫀

While explaining concentric vs eccentric hypertrophy, I initially said “end systolic volume decreases” and later corrected it to “actually it increases” in advanced DCM.

What I meant:

👉 In concentric hypertrophy (pressure overload like HCM/hypertension):
- thick ventricle
- smaller chamber
- systolic emptying may still be good initially
- ESV may remain low/normal

👉 But in eccentric dilation / end-stage DCM:
- ventricle becomes weak and dilated
- pumping fails
- more blood remains inside after systole
- therefore End Systolic Volume (ESV) becomes HIGH

So:
Big heart ≠ strong heart.

A massively dilated ventricle often means severe systolic failure with poor forward stroke volume.

That’s the beauty of cardiology physiology, understanding the mechanism matters more than memorizing terms. 🩺📚

Dog bite wounds should NEVER be ignored.
Even a small puncture wound can lead to severe infection, tissue damage, abscess formation, nerve injury, or life-threatening complications if not treated properly.

Immediate first aid matters:
• Stay calm
• Wash wound thoroughly with running water & soap
• Control bleeding gently
• Avoid home remedies
• Seek medical attention immediately
• Rabies and tetanus evaluation is extremely important

For pets:
• Veterinary examination is necessary even if the wound looks small externally
• Deep tissue pockets and hidden trauma are common in bite injuries

Early wound management can save life, limb, and prevent long-term complications.
