This agreement is between Three Dog Bakery and the pet owner whose signature appears above. 1. PAYMENT FOR SERVICES: Owner agrees to pay the rate for boarding in effect on the date the pet is checked into the kennel, as well as special services requested for the pet during the time it is in the care of the kennel.
2. VETERINARY CARE: Owner agrees in the event their dog appears to be ill or at significant risk of experiencing a medical problem, Three Dog Bakery will attempt to contact them for instructions prior to seeking veterinary care. If they cannot be reached, they agree that Three Dog Bakery may use its reasonable discretion in seeking veterinary care on their behalf and they will be responsible for all related expenses. Owner authorizes Three Dog Bakery to use the veterinarians of its choice. Owner agrees that Three Dog Bakery will not be
liable for the actions and decisions of the veterinarian. Owner also agrees to be responsible for any fees assessed by Three Dog Bakery for emergency care and transportation.
3. INJURIES & ILLNESS: At Three Dog Bakery, we strive to provide your dog with excellent care and a fun experience during their stay. Although animals are supervised at all times, injuries can still occur while dogs are playing together such as scratches, cuts or strains. Such injuries are rare, but they happen at all doggie daycares. In addition, while infrequent, dogs can get into fights, even under close supervision. Common areas for bites as a result of these fights are the snout, ears, tuft of the neck and paws. At Three Dog Bakery, we have procedures to screen dogs for aggressive behavior and we do not allow aggressive dogs to play with other dogs. However, even the friendliest of dogs can get into fights with very little or no warning. In addition to injuries, it is possible for dogs to transfer illness such as upper respiratory infections and kennel cough, even with the required vaccinations and boosters. This is just like at a daycare for children where illness such as pink eye and the flu can be transferred from one child to another. Such illnesses do not occur often and all dogs must have the necessary vaccinations to check- in.
4. AGE & VACCINATIONS: Dogs of all ages are allowed at Three Dog Bakery as long as they meet the facility’s criteria of vaccinations. The owner specifically represents to the kennel that the pet has not been exposed to rabies, parvo, or distemper within 30 days prior to current services, and further, said pet has received a current rabies (if older than four months), distemper, parvo, and bordetella vaccination which can be confirmed by the veterinarian listed on the permanent computer or the original boarding contract.
5. ABANDONMENT OF ANIMALS: Owner understands if they do not pick up their dog by 5 days after the agreed upon pick up date, a certified letter will be sent regarding the animals abandonment. Should the animal not be removed within the specified time the owner hereby relinquishes all claims to their animal, but will not be relieved of their contractual liability of any treatment, boarding or care furnished.
6. LIABLITIES: Owner agrees that Three Dog Bakery will not be liable for any claims of injury, illness, damage or death to their dog during its stay and that under no circumstances will Three Dog Bakery be liable for consequential damages. Owner certifies that they have informed Three Dog Bakery of all dog and human aggression. Owner agrees that they are responsible for any harm caused by their dog while in the care of Three Dog Bakery. Owner shall indemnify Three Dog Bakery against any claims made against it or for losses or damages suffered by Three Dog Bakery as a result of their dog.
7. PERSONAL BELONGINGS: The owner understands that the Three Dog Bakery cannot be held responsible for lost, dirty, damaged, or destroyed belongings.
8. FLEAS & TICKS: Any pet arriving with fleas or ticks will be treated at the owner’s expense prior to boarding visits.
9. SERVICE: Three Dog Bakery reserves the right to refuse services at any time for any reason regardless of reservations or prior arrangements.
