Ball Jai🐶小腸氣手術👩🏻‍⚕️✅

Ball Jai 是一隻 10 歲已絕育的雄性博美犬🐶因排尿困難，晚上前往VEC急症就診。

檢查時，我們可以感覺到 Ball Jai 左側有腫脹，稱為小腸氣（會陰疝）。

這是直腸一側肌肉上的一個小孔。X 光顯示膀胱位於腫脹的疝氣內，促使 Ball Jai 難以排尿。手術前，我們為牠檢測到心臟有雜音，超聲波檢查顯示牠患有輕度心臟病，仍然可以安排手術。

Dr. Hannah 為Ball Jai 進行了小腸氣（會陰疝）手術。直腸一側的大腫塊被切開，之後把膀胱等部位放回腹部的正常位置。然後再用附近的肌肉組織填補小腸氣（會陰疝）的缺口。

手術非常成功，Ball Jai 恢復得很好，再次開始正常排尿❤️

小腸氣通常發生於未有絕育的老年狗狗身上，但這次 Ball Jai 的例子是非常罕見🔺因為牠已經在幼犬的時候絕育了

Ball Jai 🐶- Perineal Hernia Surgery👩🏻‍⚕️✅

Ball Jai is a 10 year old castrated male Pomeranian🐶. He presented to VEC at night to the emergency department because he was struggling to urinate. On examination, the vet could feel a swelling called a perineal hernia on the left side. This is a hole in the muscles to the side of the re**um. X rays showed the bladder was inside the swelling of the hernia which was making it difficult for the dog to pass urine.

Ball Jai also had a heart murmur detected, an echo examination showed a low grade heart disease so it was ok to proceed with surgery.

Ball Jai underwent perineal hernia surgery. The big swelling to the side of his re**um was opened and the hernial contents including the bladder was placed back into their normal position in the abdomen. The hernia was repaired by lifting up a muscle from the pelvis and using it as a flap to repair the hole.

Ball Jai recovered very well and started urinating normally again. ❤️

Perineal hernia’s occur most commonly in older male entire dogs. There is a connection to testesterone so castration is recommended at the surgery if it has not been done already. Ball Jai was a bit unusual as he was already castrated when young.

🧐IBD的治療方式🐶🐶

✅飲食療法
• 水解蛋白處方飼料
• 新蛋白飼料（像鴨肉、兔肉，貓咪沒吃過的蛋白來源）
• 完全避免一般飼料、小魚乾、零食，避免干擾診斷與治療
✅消炎／免疫抑制藥物
• Prednisolone（類固醇）：最常用，抑制免疫反應與發炎
• Chlorambucil：當類固醇不夠時會加用
• 有些貓咪用藥後症狀會穩定很久
✅補充劑

🧐IBD 常見的發炎部位 🐱🐶

小腸（最常見）
• 特別是迴腸（ileum）這段，是小腸最後的部分
• 主要負責吸收維生素 B12（鈷胺素）和營養
• 發炎會導致：
• 慢性嘔吐
• 體重減輕
• 慢性腹瀉（軟便、黏便）

大腸（結腸）
• 主要負責吸收水分與形成糞便
• 如果是大腸型 IBD，會出現：
• 頻繁便意（一天拉很多次）
• 排便困難但量少
• 黏液便、血便
• 有時會發出哀號或蹲廁所很久

胃（胃炎）
• 雖然比較少見，但也有 IBD 影響到胃的情況
• 表現為：
• 嘔吐（尤其是在空腹時）
• 胃部不適、嗜睡、食慾不穩

全腸型（小腸 + 大腸 + 胃）
• 有些嚴重或長期的 IBD 會擴散影響整個腸胃道
• 會出現多種症狀混合，像是：
• 嘔吐 + 肚瀉 + 體重下降 + 食慾低下
• 更難穩定，通常要結合飲食、藥物、B12 補充一起管理

🧐Commonly Affected Areas in IBD for Cats and Dogs 🐱🐶

Small intestine (most common):
- Especially the ileum, which is the last part of the small intestine.
- Responsible for absorbing vitamin B12 (cobalamin) and nutrients.
- Inflammation can lead to:
- Chronic vomiting
- Weight loss
- Chronic diarrhea (soft stool, mucus stool)

Large intestine (colon):
- Mainly responsible for absorbing water and forming f***s.
- If it's the colon-type IBD, symptoms include:
- Frequent defecation urges (pooping multiple times a day)
- Difficulty in defecation but small amounts
- Mucus stool, blood stool
- Sometimes crying or prolonged squatting during bowel movements

Stomach (gastritis):
- Less common, but there are cases where IBD affects the stomach.
- Symptoms include:
- Vomiting (especially when fasting)
- Stomach discomfort, lethargy, unstable appetite

Entire intestinal tract (small intestine + large intestine + stomach):
- Severe or prolonged IBD may spread, affecting the whole digestive system.
- Symptoms are a combination, such as:

當寵物發生什麼狀況，需要第一時間前往獸醫診所就診？

▫️呼吸困難
如果寵物持續快速呼吸、張口呼吸、用腹部大力呼吸（沉重的呼吸），應盡快將寵物送往獸醫處理。這可能由於許多不同原因引起，務必盡快查明原因並由獸醫快速提供治療。

▫️中暑
這種情況在巴哥犬、法國鬥牛犬、英國鬥牛犬或其他短鼻犬種中更為常見。由於這些犬種無法像其他犬種一樣高效散熱或調節體溫，尤其是在炎熱天氣裡，牠們可能會因中暑而虛脫、呼吸困難甚至失去意識。如果發生此情況，應立即開始給寵物降溫，並迅速前往就診。

▫️尿道阻塞
這種情況更常見於雄性貓。需要注意的症狀包括：用力排尿但沒有尿液、感到疼痛不適、活力降低、食慾減少或完全喪失，以及嘔吐。這會導致膀胱內尿液堆積，進一步危害腎臟和心臟功能，而且這是一種非常痛苦的情況。務必立刻尋求獸醫協助！

▫️外傷
不幸的是，意外總是有可能發生。無論是交通事故、高處墜落或犬隻打鬥，都可能導致外部或內部傷害。如果發生此類情況，請立即將寵物送往獸醫檢查。

Watch out for these emergency conditions in pets that require urgent veterinary attention!

▫️Breathing difficulties
Any persistently fast breathing/ open mouth breathing (in cats)/ breathing with abdominal effort (heavy breathing) means that pets should see a vet as soon as possible. This could be due to a lot of different reasons and it is essential to find out the cause and receive treatment by a vet quickly.

▫️Heat stroke/ BOAS crisis
More common in breeds such as Pugs, French Bulldogs, English Bulldogs or any dog breeds with a flat nose and snout. This is because these breeds could not dissipate body heat or regulate their body temperature as efficiently as others. Especially during hot weather, they can collapse from heat stroke, struggle to breathe and lose consciousness. If this happens, start by cooling your pet immediately while heading straight to a vet!

▫️Urethral obstruction – more commonly seen in male cats. Signs to watch out for: straining to urinate with no urine produced, pain and discomfort, lack of energy, reduced or no appetite and vomiting. This condition means that urine in the bladder builds up and this risks worsening kidney and heart function. It is also a very painful condition. Immediate veterinary attention is required!

▫️Trauma
unfortunately accidents happen sometimes. Whether it’s road traffic accident, fall from height or dog fight, injuries can occur externally and internally. If any of these happen, head straight to a vet.

🧐為甚麼會發生IBD 🐱🐶?

其實為甚麼會發生 IBD 到現在醫學上還沒有完全確定單一原因，但有幾個被廣泛認可的可能因素，而且通常是多個因素一起作用才會導致 IBD 的發展。

✅食物過敏／不耐受
• 有些貓狗的免疫系統會錯誤地把某些食物成分（常見是蛋白質）當成「敵人」來攻擊。
• 長期吃同一種蛋白質（如雞肉、魚肉）可能導致免疫過敏反應，刺激腸道黏膜，引起慢性發炎。
✅腸道菌叢失衡（Dysbiosis）
• 健康的腸道中有大量「好菌」與少量「壞菌」共存。一旦腸道菌叢比例失衡（例如：抗生素用太多、飲食不當），壞菌會增生，破壞腸道黏膜，引發免疫反應與發炎。
✅自體免疫問題
• IBD 本質上就是一種免疫系統錯亂的疾病。正常情況下，免疫系統應該只攻擊真正的病菌或毒素。但在 IBD 的貓狗體內，免疫系統會錯誤攻擊正常的腸道細胞或腸道內正常細菌，導致發炎。
✅慢性腸道感染
• 例如曾經感染過寄生蟲（如絛蟲、梨形鞭毛蟲）或某些細菌，這些感染可能會觸發免疫系統「記憶錯誤」，留下長期發炎反應。雖然病原體可能已經清除，但發炎仍持續。
✅遺傳體質
• 某些品種的貓（如緬因貓、暹羅貓）、狗（如德國牧羊犬、拉布拉多犬）似乎較容易發生 IBD，這表示可能有基因相關性。
✅壓力與環境改變
• 長期壓力（如搬家、噪音、陌生人）會影響腸道蠕動與免疫功能。
• 腸道是貓狗壓力最容易反應的地方之一，很多貓狗壓力大會肚痾及食慾變差。

總結來說，IBD 的發生通常不是單一原因，而是：食物過敏 + 腸道菌異常 + 免疫過度反應 + 壓力 +（可能）遺傳這些複合起來，才導致慢性腸道發炎。

🧐Why does IBD occur? 🐱🐶

✅Food allergies/intolerances
- Some pets’ immune systems mistakenly identify certain food components (commonly proteins) as “enemies” and attack them.
- Long-term consumption of the same protein (such as chicken or fish) may trigger immune reactions, irritating the intestinal lining and causing chronic inflammation.

✅Imbalance of gut microbiota (Dysbiosis)
- A healthy gut hosts a large number of “good bacteria” alongside a smaller quantity of “bad bacteria.” If the balance of gut microbiota is disrupted (e.g., due to excessive antibiotic use or poor diet), bad bacteria can proliferate, damaging the intestinal lining and triggering immune responses and inflammation.

✅Autoimmune issues
- IBD is essentially a disorder of the immune system. Normally, the immune system should only attack actual pathogens or toxins. However, in pets with IBD, the immune system erroneously attacks normal intestinal cells or gut flora, leading to inflammation.

✅Chronic intestinal infections
- For instance, previous infections with parasites (like tapeworms or Giardia) or bacteria may mislead the immune system’s “memory,” leaving behind persistent inflammation. Even after the pathogens are eliminated, the inflammation continues.

✅Genetic predisposition
- Certain cat breeds (like Maine C***s and Siamese cats) or dog breeds (like German Shepherds and Labradors) appear more susceptible to IBD, indicating a possible genetic link.

✅Stress and environmental changes
- Long-term stress (e.g., moving homes, exposure to noise, meeting unfamiliar people) impacts intestinal movement and immune function.
- The gut is one of the areas most sensitive to stress in pets—many cats and dogs experience diarrhea and appetite changes under stress.

In summary, IBD usually results from a combination of factors, such as food allergies, gut dysbiosis, immune overreaction, stress, and possibly genetics—all working together to cause chronic intestinal inflammation.

🧐 甚麽是 IBD? 🐱🐶

炎症性腸病（Inflammatory Bowel Disease）是一種腸道長期發炎的慢性疾病，影響的是胃腸道，主要是小腸或大腸。

腸道的免疫系統本來是要保護身體，但在 IBD 的情況下，它誤判了正常的食物或腸道內的細菌，結果產生了持續性的發炎反應。這種發炎會影響腸道的功能，導致消化吸收異常。胃腸道長時間處於輕微或中等程度的發炎狀態，而且這種發炎不是短期腸胃炎那種能自己好的，而是需要長期治療與飲食管理。
