Remedial exercises - Sit-to-Stand

Remedial exercises are arguably the most important part of the physiotherapy plan, especially when considering rehabilitation and recovery post injury. During the physiotherapy session we aim to assess any muscle imbalances or weaknesses and address those with specific, targeted exercises. We will go through the step by step process of teaching your dog the exercise, the aim of it, what ypu need to look out for, specifics like paw placement and muscle engagement etc.
In this particular exercise we are aiming to strengthen the quadriceps and activate the gluteal and hamstring muscles. To achieve that the dog must keep the hind paws in the same position while the forelimbs move backwards and forwards, aiming for a square sit and a sqaure stand. Just like when we go to the gym we are looking for correct form and slow, controlled movement to achieve the best results. This is the foundation exercise to a lot of remedial exercise programmes

Spirit really finding some release in the acupressure during his session, this is a horse that rarely relaxes fully during physio and will eat constantly if hay is available. And he's choosing to stand completely relaxed and process the treatement
I see spirit to help manage compensations from previous injuries and arthritis

Haps loving his laser treatment 😴

Haps has been a regular client as we manage his spinal conditions and suspected FCE. Haps was completly paraplegic before starting treatement and with the use of combined physiotherapy and hydrotherapy we have been able to get him up and walking again, though at his own pace πŸ˜… He still enjoys his regular treatments aiming to manage compensations and maintain comfort and mobility

Basha absolutely loving his TENS treatment 😴 This guy had some issues with chronic back pain which after a change of saddle and careful management with massage and electrotherapies as well as remedial exercise over multiple months, has improved significantly. A turn from winter to spring can be challenging for him though which is when we pull out some of the electros again to help him feel his best

TENS or Transcutaneous electrical nerve stimulation uses skin surface electrodes to transmit electrical impulses which excites the sensory nerves providing pain relief in two different ways:
Β· Stimulating the pain gait mechanism
Β· Inducing an opioid release
Stimulating the pain gait mechanism will decrease the transmission of pain through stimulating faster A beta fibres which will block the signals of the slower C fibres from transmitting pain stimuli.
Stimulating A delta fibres will lead to endogenous opioid release, releasing the opioids into the spinal cord and decreasing the release of neural transmitters thus blocking the transmission of pain

TENS can:
βœ…Reduce pain
βœ…Aid in relaxing muscles
βœ…Improve blood flow
βœ…Improve healing rates
βœ…Allow for implementing remedial exercise
βœ…Break the pain cycle

TENS cannot:
❌Solve primary issues
❌Resolve injuries
❌Build muscle

Let's talk gait abnormalities
Generally dogs will display three gaits - walk, trot and canter - with walk being a four beat gait, meaning all limbs will move separately in a set pattern and there will always be at least one foot on the ground. However, occasionally you will see an abnormal movement such as a pace where the ipsilateral limbs (limbs on the same side of the body) will move in unison at the same time with no suspension phase.
While this movement may be more common for certain breeds it is not considered normal for a dog to move consistently in this way. If you notice your dog chooses to pace it may indicate a musculoskeletal or orthopaedic discomfort higher in the body or may be a result of previous injury. Discomfort in the hip and lower back as well as pelvic limb and ventral lumbar muscles are some of the common causes of consistent pacing in dogs

Puzzle (very reluctantly) showing he does actually enjoy his treatment, peep the lip going constantly πŸ˜… he's a very fidgety and busy boy but does show a lot of lovely yawning and chewing, he just also kicks the door at the same time 🀣 he's a character and hopefully will be feeling much better now after his last few sessions and he can enjoy his work again

Pulsed Electromagnetic Field therapy is a therapy where an electromagnetic field is emitted at a specific frequency and rate that optimised body’s ability to repair and re-establish the normal cellular voltage. LASER or photobiomodulation is the use of red and infrared light energy of a specific wavelength in order to combat pain and inflammation as well as improve the healing rates and the quality of new tissue growth.
The light energy gets absorbed by the chromophores within the cell’s mitochondria affecting the disassociation and release of Nitric Oxide NO causing:
β€’ increased blood flow - vasodilation
β€’ decreased pain
β€’ improved mobility
β€’ modulation of inflammatory mediators
β€’ increased ATP production - improved cell metabolism
β€’ increased fibroblasts and macrophages production

The laser light is able to inhibit the transmission of pain through inhibiting the Adelta and C pain fibres carrying a noxious (harmful) stimuli. As well as causing an endogenous opioid release causing the release of serotonin and endorphins and thus inhibiting substance P responsible for the transmission of pain
