ANVH EP- 93 Foreign body in cat 貓咪吞針

Today A Niu would like to introduce a classic emergency case - a cat swallowed a needle - it is urgent, the vet needs to get involved ASAP condition

A swallowed sewing needle is an emergency. The needle can puncture the stomach or intestines, and the attached thread can cause the intestines to bunch up and tear.

🚨 Possible Signs
Vomiting or gagging/Not eating/Lethargy/Drooling or difficulty swallowing and Abdominal pain
(Some cats show no symptoms at all.)

❌ Do NOT
Do not induce vomiting
Do not pull the thread
Do not wait to “see if it passes”
Do not feed large meals to “push it through”

✔️ What You Should Do
Go to a veterinarian immediately
Tell the vet what the cat may have swallowed and when
Bring the same type of needle/thread if available

🩺 What the Vet Will Do
X‑rays to locate the needle
Ultrasound if a thread is involved
Removal by endoscopy (if in the stomach) or surgery (if in the intestines)
Treatment for complications if perforation has occurred

👉 這不是觀察就好的狀況，必須立即就醫。

🚨 可能出現的症狀（但有些貓完全沒症狀！）
嘔吐或乾嘔/食慾下降、不吃飯/腹痛、精神差/流口水、吞嚥困難/排便減少或帶血等，但即使沒有症狀，也不能排除危險。

🩺 為什麼不能等？
針可能在體內移動，越久越容易刺穿腸胃道
線會讓腸子像「手風琴」一樣被拉扯 → 腸壞死、穿孔、敗血症
早期取出成功率高、併發症低
晚期可能需要 腸切除手術，甚至有生命危險

❌ 飼主千萬不要做的事
不要催吐（針可能刺破食道）
不要拉線（會造成腸子撕裂）
不要觀察一兩天再說
不要餵食大量食物想「包住針」（無效且危險）

✔️ 飼主應該立刻做的事
盡快帶去 動物醫院或急診

告知醫師：可能吞下的物品（針、線、長度）以及何時發生
若能安全取得另一端的線或針的樣子，可帶去給醫師參考

🩻 醫院會怎麼處理？
X 光檢查：大多數縫衣針可被看到
超音波：評估線性異物、腸道狀況

治療方式依位置決定：
在胃內 → 可能可用 內視鏡取出
在腸道 → 多數需要 手術取出
若已穿孔 → 可能需 腸切除 + 治療腹膜炎等等

這個case很幸運的針卡在口腔下顎,經鎮靜後取出,沒有造成太大的傷害,但是主人嚇壞了

🐶 Mammary Tumors in Dogs — What Owners Should Know
Mammary gland tumors are very common in unspayed female dogs.

1. About 50% can be cancerous.
2. Look for small or large lumps along your dog’s belly or ni**les.
3. Early check‑ups and early surgery give the best outcome.
4. Spaying before the first heat reduces the risk to almost zero.
5. If you find a lump, don’t wait — book a vet exam ASAP !!

The only reason for not spaying is breeding, if you don't consider breeding, then spay your pets ASAP

ANVH EP- 91 Perianal hernia in dogs - 狗的會陰疝氣

Today A Niu would like to introduce a case of Perianal Hernia in dogs

Perineal hernia in dogs is a condition where the pelvic diaphragm muscles weaken or fail, allowing pelvic or abdominal organs (fat, re**um, bladder, prostate, or intestines) to herniate into the perineal region. It occurs primarily in older, intact male dogs, and often presents with straining, constipation, and a soft swelling beside the a**s.

🐕 Who gets it
Middle‑aged to older intact male dogs are overwhelmingly affected.

Breeds with reported predisposition include Corgis, Boston Terriers, Boxers, Collies, Dachshunds, Miniature Poodles, Old English Sheepdogs, and Pekingese.

🔍 Clinical signs
Most dogs show one or more of the following:

Soft, reducible swelling beside the a**s (unilateral or bilateral)
Tenesmus (straining to defecate)
Constipation or obstipation
Dyschezia (pain/difficulty defecating)
Stranguria/dysuria if the bladder is involved
Acute collapse if the bladder becomes trapped and obstructed (true emergency)

🧪 Diagnosis
Diagnosis is based on:
Re**al exam → reveals weakness of pelvic diaphragm, re**al deviation, or sacculation
Palpation of herniated structures
Radiographs or ultrasound → identify bladder, prostate, bowel loops, or fat within the hernia

🛠️ Treatment
Stabilization (if emergency)
Definitive treatment: Surgery
Surgery is the gold standard and involves:
Reducing herniated organs
Reconstructing the pelvic diaphragm using:
Internal obturator muscle flap (most common)
Superficial gluteal or semitendinosus flaps in severe cases
Castration at the same time → reduces recurrence risk by ~3×

會陰疝氣是指 骨盆隔肌（levator ani、coccygeus 等）變弱或破裂，導致腹腔或骨盆腔的器官（脂肪、直腸、膀胱、攝護腺、腸道）突出到肛門旁的會陰區域。最常見於 中老年未結紮公犬。
常見品種：柯基、波士頓、拳師、牧羊犬、臘腸、貴賓、北京犬等

🔍 臨床症狀
肛門旁 柔軟、可推回的腫脹（單側或雙側）
排便困難、用力排便（tenesmus）
便祕、糞便變細
排尿困難（若膀胱被捲入)
急性無法排尿 → 生命危急（膀胱被困住）

🧪 診斷方式
直腸指診：可觸到骨盆隔肌薄弱、直腸偏移或囊袋形成
X 光或超音波：確認是否有膀胱、腸道或脂肪突出
血檢、尿檢：評估是否有脫水、腎損傷或尿滯留

🛠️ 治療方式
根本治療：手術修補
手術是唯一有效的治療方式：
將突出器官復位-->修補骨盆隔肌
常用術式：內閉孔肌瓣（internal obturator muscle transposition）
嚴重病例：可用臀肌或半腱肌瓣

👉 同時結紮非常重要，可降低復發率約 3 倍。

ANVH EP-88 Demodex infection 毛囊蟲感染

Today A Niu would like to share a classical but rare case - Demodex infection

Demodex (demodectic mange) is caused by mites that normally live on a dog’s skin. Most dogs stay healthy — problems happen when the immune system can’t keep the mites in check.

What it looks like
Patchy hair loss (often around eyes, muzzle, legs)
Red, scaly skin
Odor or crusting if infection develops
Usually not very itchy unless there’s a secondary infection

Who gets it
Puppies: small, round bald spots that often clear on their own
Adults: may signal an underlying issue (hormonal disease, cancer, stress, malnutrition)

Is it contagious?
❌ Not contagious to people
❌ Not contagious to other adult dogs
✔️ Puppies get mites only from their mom shortly after birth

Treatment
Modern flea/tick meds (isoxazolines like Simparica, NexGard, Bravecto) are highly effective

Treat secondary infections if present

Avoid immune‑suppressing meds like steroids or Apoquel

Prognosis
Localized cases: excellent
Generalized cases: good with proper treatment
Adult-onset: depends on finding the underlying cause

❌ 不會傳染給其他成犬
✔️ 幼犬只會在出生後不久從母犬身上獲得蟎蟲

治療方式
新一代跳蚤壁蝨藥（如 Simparica、NexGard、Bravecto）效果非常好,但若有細菌感染需同時治療

避免使用會抑制免疫的藥物（如類固醇、Apoquel）

預後通常非常好,但嚴重的全身感染需要較長時間的治療
成犬發病：需找出背後的免疫或內科疾病

ANVH EP-87 Traumatic proptosis in dogs 創傷性眼球出

狗的創傷性眼球突出（Traumatic Proptosis in Dogs）

Traumatic proptosis refers to displacement of the globe beyond the eyelid margins following external trauma. The eyelids become trapped behind the globe, resulting in exposure, vascular compromise, and potential extraocular muscle avulsion. It is considered an ophthalmic emergency requiring immediate intervention.

Common cause include:
• Dog fights or dog–cat altercations
• Motor vehicle trauma, falls, blunt head injury
• Brachycephalic breeds predisposed due to shallow orbits

Clinical sign:
• Globe protrusion with inability to blink
• Corneal desiccation, ulceration, exposure keratopathy
• Marked conjunctival edema or hemorrhage
• Extraocular muscle avulsion (commonly medial re**us)
• Optic nerve stretching or transection

Treatments include:
1. Globe Replacement + Temporary Tarsorrhaphy
Indicated when the globe remains viable, the cornea is intact, and the optic nerve is not severely compromised. Under general anesthesia, the globe is lubricated, repositioned, and protected with temporary tarsorrhaphy.

2. For more severe case, may need to do Enucleation
Recommended when the globe is ruptured, the optic nerve is transected, multiple extraocular muscles are avulsed, or vascular supply is lost. Enucleation prevents chronic pain and infection.

• 結膜嚴重水腫或出血
• 眼外肌撕裂（尤其是內直肌）
• 視神經牽拉或斷裂

治療選項
1. 眼球復位與暫時性瞼縫合
適用於眼球仍具血供、角膜完整、視神經未完全損傷者。
在全麻下以潤滑後復位，並以部分瞼縫合保護眼球。

2. 眼球摘除術
若眼球破裂、視神經斷裂、多條眼外肌撕裂或無血供，建議直接摘除以避免疼痛與感染。

這個case在經過治療後,我們保留了這顆眼睛,目前正在逐漸恢復當中

ANVH EP-86 Aural hematoma in dogs - 狗的耳血腫

Aural(ear) hematoma in a dog is essentially a pool of blood trapped under the skin or within tissue, most often caused by ruptured small blood vessels(scratching or shaking the head).

Common signs
• Soft, fluid‑filled swelling
• Ear looks puffy, warm, or painful
• Often linked to ear infections, allergies, or injury

Will it heal on its own?
• Small ones may absorb
• Ear hematomas rarely heal well alone and can lead to a “cauliflower ear”

What treatment involves
• Draining or surgical stabilization
• Treating the underlying cause (ear infection, allergies, parasites)
• Pain and inflammation control

When to see a vet
• Rapid swelling
• Significant pain or heat
• Bad smell or discharge
• Low energy or worsening symptoms

ANVH EP-84 Splenctomy tumor in dogs - 狗的脾臟腫瘤摘除

This 8‑year‑old Golden Retriever came to us after a few days of not eating and feeling very low in energy. Tests showed severe anemia and low platelets, and he was eventually brought to A Niu for further care.
After discussing the options with his family, we proceeded with a splenectomy to give him the best chance of recovery.
He did great after surgery and is now back home resting comfortably — ready to enjoy life and keep going strong for many more years.

🐶 Splenectomy in Dogs - A splenectomy is the surgical removal of the spleen. It’s a common and often life‑saving procedure for dogs, especially when the spleen is bleeding or has developed a mass.

Why a Dog Might Need a Splenectomy
• Splenic masses (benign or malignant)
• Internal bleeding (hemoabdomen)
• Splenic torsion
• Trauma‑related rupture
Even benign masses can rupture and cause dangerous bleeding, so surgery is often recommended.

🩺 What the Spleen Does
The spleen helps filter blood and support the immune system, but dogs can live normal lives without it. Other organs take over its functions.

🛠️ What Happens During Surgery
• Stabilization (fluids, blood products if needed)
• Abdominal surgery to remove the spleen
• Checking for bleeding or spread of disease
• Biopsies if abnormalities are found

術後恢復狀況良好,目前已經返家休養,再戰5年
