I've seen a few small or end stage kidneys in cats in my time, but I think this one takes the cake. This is an ultrasound clip of a kidney that measures 1.4cm in length and 0.7cm in width. 🤯

As promised, here are the radiographic and CT images from the same patient with gas noted in the hepatic parenchyma on ultrasound. Radiography confirmed that the echogenicities were due to gas (and there was also free gas in the peritoneal cavity), and a CT was performed for surgical planning. The arrowheads indicate the location of the free gas in the liver and also in the peritoneal cavity on the radiographic image.

Hyperechoic speckles in the liver parenchyma are highly suggestive of gas within the parenchyma which is concerning for hepatic abscessation. Mineralisation can also create hyperechoic specks - if in doubt, take an abdominal radiograph (or CT!). Radiographs and CT to follow in further posts.

Not all tubes in the liver are blood vessels! These are distended (and thickened) intrahepatic bile ducts in an 8yr Russian Blue with suspected biliary tract inflammation and obstruction. Colour flow Doppler US can be used to distinguish between distended intrahepatic bile ducts and vessels.

Following on from the what’s in a neck post....unfortunately this is something we see too often, penetrating stick injury! This one was rather spectacular, the stick had penetrated the oral cavity and lodged itself on the right side of the trachea and oesophagus. This was removed at surgery.

One of the challenges we face in MRI of the small animal brain is to differentiate between clinically irrelevant ('congenital') ventriculomegaly, which is seen very commonly in dogs, and clinically relevant ('obstructive') hydrocephalus of the brain. Here are some recent examples we have seen at NOVIS of cases of acute hydrocephalus related to the presence of intraventricular or periventricular tumours.

Some of the characteristics we look for, as indicated below in the pictures, include:
- peri-ventricular oedema on the FLAIR images (FLAIR = fluid attenuated inversion recovery, which suppresses the signal from pure fluids including CSF); shown as white rims to the ventricles
- elevation of the corpus callosum, as can be seen on the sagittal T2W sequence (where CSF is not suppressed and is usually bright white); note how rounded the ventricle appears
- flattening of the interthalamic adhesion (which is surrounded by the ventricles and becomes narrowed when they distend)
- and some other signs

These were two recent cases of suspected canine brain tumours, one being a choroid plexus tumour and the other being a peri-ventricular glioma. Enjoy!
