Introducing Bear, 9YO male neutered handsome Labrador. Bear was referred to Cedar Mountain Vet Referral for a suspected spinal condition. Bear apparently developed non-ambulatory hind quarter paraparesis (could not move his hind legs). Previously, he was running without problem. Bear could not walk at the time of presentation, although he could stand up with ventral support. His lower back elicited pain, but thorough neurologic examination revealed neurologic function was sound. Orthopaedic examination revealed bilateral hip pain. Differentiation between lumbosacral disease and hip pain is challenging. Radiographs from the referring veterinarian showed acceptable level of hip congruity and coverage (not dysplastic) but Morgan signs. Examination under sedation revealed severe hip laxity and positive Ortolani sign. Bilateral hip joint pathology was suspected: overstretch (sprain) of hip joint capsule and/or capital ligament and/or hitting femoral head to the acetabulum, causing trauma and pain to his hips due to subluxation of the joint while he was running. Because the hip joint changes were mild, total hip replacement or femoral head and neck excision was not offered. Conservative management with extensive acute pain control in hospital and rehabilitation were recommended. Shockwave therapy was initiated. Thanks to our anaesthetist registrar Dr Dave for his extensive knowledge, with extensive pain management, Bear started to walk by himself next day. Bear was discharged to the owner 2 days later with full ambulation. Bear came back for rehabilitation the following week. Our rehabilitation veterinarian Dr Fran made an individual program for Bear. The video shows how good and strong he is now. This acute event occurred less than 2 weeks ago, when he was completely non-ambulatory. Now Bear got back to himself, enjoying his walk.
