If you are still wanting to get on our waitlist please send me your email and I will send you a sample contract. To get on the list you must return the contract with your name, ph # and address along with a vet reference.

I have sent out many contracts but only a handful have been returned. I will stop accepting request to be on the wait list on 2/14 so if you’ve received a contract please return prior to the above date.
