🩺 Case of the Month — Bay Area Vet MRI

Case: French Bulldog with progressive vestibular disease

This month’s featured case highlights the importance of advanced imaging and diagnostic testing in identifying complex neurological infections.
A French Bulldog was referred to Bay Area Vet MRI with an eight-day history of progressive vestibular disease and loss of appetite.

💡 MRI Findings:
MRI scans revealed signal abnormalities in the left brainstem and left tympanic cavity, with contrast enhancement along the meninges and petrous bone—findings consistent with infection extending from the middle/inner ear into the brainstem.

🧫 Additional Tests:
CSF analysis showed severe neutrophilic inflammation, with cultures identifying E. coli (resistant to amoxicillin-clavulanate, sensitive to enrofloxacin).
Myringotomy fluid also confirmed septic inflammation with phagocytized rods and hemorrhage, supporting a diagnosis of septic meningoencephalitis/otitis due to E. coli.

💊 Treatment Plan:
Immediate anti-inflammatory therapy (DexSP IV) was administered, followed by antibiotic treatment with Enrofloxacin and Amoxicillin-Clavulanic Acid pending culture results.
A short course of prednisone and gabapentin was added to manage inflammation and discomfort.

✅ Diagnosis:
Left middle and inner ear infection extending into the left brainstem (septic meningoencephalitis/otitis due to E. coli).

🧠 Advanced MRI and laboratory diagnostics played a crucial role in identifying the infection source and guiding targeted treatment.

🐾 Case Highlight: 10-Year-Old German Shepherd 🐾

This sweet patient came to us with weakness in the hind limbs. To better understand what’s going on, we performed a thoracolumbar (TL) spinal MRI.

✨ MRI allows us to see detailed images of the spinal cord, vertebrae, and surrounding tissues — helping veterinarians pinpoint the cause of mobility changes such as weakness, pain, or difficulty walking.

🧠 Advanced imaging is one of the best tools we have for guiding diagnosis and treatment, giving pets the best chance at improved comfort and mobility.

Buckle up for a great success story! 🍀Meet Lucky🍀. Lucky is a sweet 11 year old M/N Chihuahua that presented to Bay Area Vet MRI for 6 weeks of paraparesis which was non responsive to Meloxicam & Gabapentin. It progressed to paraplegia two days prior to coming in for the MRI. A spinal MRI revealed a diffuse area of non-homogenous contrast enhancement near L3. This was suggestive of either a intra-axial spinal cord tumor or an infectious/inflammatory process. An MRI could not narrow it down completely therefore a spinal tap (CSF tap) was recommended.

The CSF tap revealed that Lucky has Autoimmune Meningo-Myelitis, an extremely rare condition in geriatric dogs.

The MRI Scan & CSF tap combined, prior to starting corticosteroids treatment, allowed prompt diagnosis and a specific and effective long term therapy.

After 4 days of prednisone therapy, Lucky showed significant improvement! Two weeks later cyclosporone was implemented & the prednisone was tapered and eventually discontinued. Lucky made a full recovery thanks to the prompt MRI Scan along with a CSF tap!

We are so happy that we were able to give Lucky and his owners the answers they needed and treatment that Lucky needed. Yay for Lucky who is very Lucky (and handsome)! 🍀😀👏
