This clinic is for dogs and cats. Dogs must be 8 weeks of age. Cats must be 2 lbs!!
1. Type of pet (cat or dog) & gender
2. Age of pet
3. Approximate weight if your pet is a dog. (We have to have an approximate weight on dogs to schedule you)
4. Your phone number.
5. If your pet has a current Rabies Vaccine.
WE WILL MESSAGE YOU BACK WITH AN APPOINTMENT TIME AND ADDRESS OF CLINIC AFTER YOU HAVE AN APPOINTMENT.
We put a lot of volunteer hours into these clinics and we want it to go very smooth.
