Well done to Dr Hyman and Dr Joyce for again being selected Northern Virginia magazine equine veterinarians of the year.

This is Dr Joyce's 7th year in a row and Dr Hyman's fourth year. Both remained very appreciative of their colleagues' and community's support.

Nominations for this distinction begin with selection by other local equine veterinarians and then there is both public and private voting.

This group makes it all work for the betterment of every horse they support. They care, they are compassionate, and they sincerely believe that every horse deserves their very best effort for you and your horse.

We were able to enjoy some "Polo at the Park" on our home turf at Morven Park.

Rain cut it a little short yet it was really wonderful to be together.

CAC Games midweek update. The horses and riders are acclimating to the heat and boarding conditions. The venue is coming together with 24-hour work crews. 1st FEI jog is tomorrow Friday at 10am.

My favorite Team Mexico Veterinarians are here and it's super great catching up with them. Their barn aisle decorations make ours look like kids play.

We shifted hotels last night into the Games hotel. Lots of teams of lots of countries here.

We're keeping the horses hydrated and happy. It's fascinating how pampered they are: massage, lasers, physio, baths, hoof care, combing and brushing, meals to order, and so much pampering. ๐Ÿ‘‘๐ŸŽ

LET'S GO BARBADOS! ๐Ÿ’ช๐Ÿ‡ง๐Ÿ‡ง๐Ÿด

TEVA and Dr Jay Joyce representing Barbados at the Central American and Caribbean (CAC) Games as Team Veterinarian for this small, yet immensely impressive and driven group of riders. I am beyond fortunate to be here amongst these athletes and friends.

Aiming to improve on our 2019 Bronze medal performance in Eventing in Bogota, Colombia, our post-covid team is even more ready!

Barbados has 2 dressage riders (Roberta Foster on Chic and Mackenzie Manning on Rue) and 2 eventers (Monique Archer on Cami and Zoe Archer on Millridge).

This competition is the precursor to the Pan Am Games in the fall, then on to the Paris 2024 Olympics.

This amazing group has already conquered travel delays, storm cancellations, administrative holds at the airport, non-potable water in stabling, dead cell phones, an under-construction venue, 1 hour Uber or shuttle rides to/from the horses, 95 degree buggy days, language barriers, and undrinkable water in Santo Domingo. The upcoming competition days will be easy compared to the hurdles overcome already.

The horses and riders remain enthusiastic, focused, and healthy. They are ready to face giants like Mexico and Colombia starting Saturday. Our underdog spirit is infectious, and the spirit of the Games continues to rise.

The majority of the CAC Games are in El Salador. Only Taekwondo, modern pentathlon, equestrian, racquetball, canoeing, field hockey and clay pigeon needed hosting by another country. Sadly, no proper athlete's village or opening ceremony for us in DR.

More to follow as the official jog is Friday, Dressage Saturday, cross country Sunday, jumping Monday evening.

Some morning continuing education at the Goodstone Inn & Restaurant with Dr. Sallie Hyman and her assistant for a couple weeks, Brooke Fletcher, and Dr Jay Joyce. We had an informative session with several other local equine veterinarians. Always great to catch up with other horse vet friends.

Thank you Dechra US for the breakfast and opportunity to learn.

Osphos, a new FDA approved Pentosan, and updates on the various regenerative joint therapies were the highlights.

Summarizing:
1. Anything derived from the horse's blood, processed onsite in the centrifuge, and injected back into the horse's joint is a gazzilion times more effective than just steroids on a million different levels.

2. We need to use Osphos more frequently than yearly- like twice a year has superior results.

3. Pentosan rocks! Recent research is very positive and it will disrupt the current intramuscular arthritis management option. Look for its release in a few weeks.

Cushingโ€™s disease, also known as Pituitary Pars Intermedia Dysfunction (PPID), affects the endocrine system of the horse. PPID is more common in older equines, with a documented prevalence of approximately 20% (1 of 5) in horses 15 years and older. Disease prevalence increases to 30% (1 of 3) in horses over 30 years of age. Horses younger than 15 can develop PPID. There is no apparent breed or s*x predilection, and the only documented risk factor for development of PPID is advancing age.

Just like heart disease, high cholesterol, and diabetes in people, earlier recognition and management of this disease is critical. Waiting until your horsesโ€™ hair coat is overly long is too late. Itโ€™s not just a long hair coat; itโ€™s not shedding out well. Also, early cases can present as poor performance or tendon/ligament injuries.

TEVA now recommends testing horses aged 15 and older. Testing for Cushingโ€™s using a TRH Stimulation Test can detect disease earlier than just a baseline ACTH Test. This test is recommended for younger horses and suspected cases that have a borderline ACTH level.

What a great group and great night at the Harry Potter: A Forbidden Forest Experience !!

Combining the TEVA, NEC, and NEFC teams was a ton of fun. We started with a Harry Potter themed potluck, followed by the walk thru the Park, then returning for post-Park fun where we started.

Winner of the Harry Potter Sorting Hat contest was Dr. Sallie Hyman who was unanimously voted as a Slytherin by all 27 present.

As a whole, it's hard to imagine a better team. A team brought together to provide you exemplary horse care - from veterinary, to emergency, to performance, to boarding, rehab, reconditioning, and fitness. Every member here is committed to you and your horse. ๐Ÿ™

INFECTIOUS DISEASE ALERT

Dr. Nathan Slovis our infectious disease chairman has been consulting with several of his colleagues in Texas and Louisiana about a PRESUMPTIVE botulism outbreak associated with Hay Cubes manufactured in Colorado.

Both Farms used the same source for the hay cubes.

We have heard that other states that MAY have farms affected as well.

The hay cubes appear to have been contaminated with dead animal carcasses (Images Attached from one of the farms affected that used the hay cubes)

Botulism

Definition/Overview

Botulism is a neuromuscular disease characterized by flaccid paralysis that is caused by neurotoxins produced by strains of Clostridium botulinum. Horses are one of the most susceptible species, with both individual and group outbreaks reported.

Etiology

Clostridium botulinum is a Gram positive, spore forming anaerobic bacterium. Spores are found in the soil throughout most of the world with the distribution of strains dependent on temperature and soil pH. Eight serotypes of botulinum neurotoxin exist and are labeled A, B, C1, C 2 , D, E, F and G, all of which have similar toxicity. There is geographic variation in the predominant serotypes. In North America, botulism in horses is most often caused by type B toxin and less often by toxin types A and C1.

Pathophysiology

There are two main forms of botulism. Toxicoinfectious botulism, also known as โ€˜shaker-foal syndromeโ€™ occurs almost exclusively foals as a result of overgrowth of C. botulinum in the intestinal tract, followed by production of neurotoxins. The disease most often affects fast growing foals from 1-2 months of age, although cases outside this age range have been seen. The mature, protective gastrointestinal microflora of adult horses typically prevents overgrowth of C. botulinum following ingestion.

In adult horses, botulism occurs following ingestion of pre-formed toxins in feed. Spoiled hay or silage are most commonly implicated in botulism caused by types A and B. Silage with a pH greater than 4.5 is favorable for sporulation and toxin production. This is known as โ€œforage poisoningโ€. It has also been suggested that birds may be able to carry preformed toxin from carrion to the feed of horses. Type C botulism is associated with ingestion of feed or water contaminated by the carcass of a rodent or other small animal. Less commonly, botulism can occur when neurotoxins are produced in wounds infected with C. botulinum. Proliferation of C. botulinum type B organisms in gastric ulcers, foci of hepatic necrosis, abscesses in the navel or lungs and wounds in skin and muscle have been associated with toxicoinfectious botulism.

The third less common form of botulism is associated with the infection of wounds with C. botulinum

Botulinum neurotoxins bind to presynaptic membranes at neuromuscular junctions, irreversibly blocking the release of the neurotransmitter acetylcholine resulting in flaccid paralysis. Botulinum neurotoxin has also been linked to equine grass sickness.

Clinical presentation

The clinical picture of symmetrical flaccid paralysis is consistent, with the onset and rate of progression dependent on the amount of toxin that is absorbed. The initial clinical signs include dysphagia with apparent excess salivation, weak eyelid tone, weak tail tone and exercise intolerance. Affected animals also spend increased amounts of time resting due to generalized muscle weakness, which is also associated with tremors, carpal buckling and ataxia. Pharyngeal and lingual paralysis causes marked dysphagia and predisposes to aspiration pneumonia. The affected animals tend to quid their food. Paralysis of the diaphragm and intercostal muscles results in an increased respiratory rate and decreased chest wall expansion. Severely affected animals die from respiratory paralysis and cardiac failure.

Differential Diagnosis

Differential diagnoses for botulism include viral causes of encephalitis, protozoal causes of encephalomyelitis, and toxic causes of sudden death or neurologic dysfunction.

Diagnosis

Botulism should be suspected in animals with flaccid paralysis displaying the above clinical signs. Botulinum toxin does not affect the central nervous system but does affect the cranial nerves; thus symmetrical cranial nerve deficits in an animal with normal mentation can help differentiate botulism from other disorders. Botulism is often a clinical diagnosis. Definitive diagnosis can be achieved by the mouse inoculation test using serum or gastrointestinal contents. However, horses are extremely sensitive to the toxin and this test is often negative. The traditional mouse bioassay identifies Clostridium botulinum in only about 30% of f***s collected from adult horses with clinical disease. If the toxin is demonstrated with mouse inoculation, the serotype can be determined through inoculation of mice passively protected with different serotypes of antitoxin. Detection of antibody titers in a recovering unvaccinated horse is also evidence for the diagnosis of botulism. Demonstration of spores in the intestine is not diagnostic, as they can be ingested and observed as contaminants.

Quantitative real-time PCR (qPCR) test for the detection of Clostridium botulinum neurotoxins in equine diagnostic samples (F***s or Food Samples) are available. . This assay tends to be more economical, time efficient and sensitive than the traditional mouse bioassay

Management

Immediate treatment with a polyvalent antitoxin prevents binding of the toxin to presynaptic membranes. However, antitoxin cannot reactivate neuromuscular junctions that have already been affected. Thus, antitoxin administration may have little effect in animals that are severely affected. Generally, only one dose (200ml of antiserum to foals (30,000 IU) or 500ml (70,000 IU) to adults of antitoxin is needed and provides passive protection for up to two months.

Antibiotics should be administered if toxicoinfectious botulism is suspected or if here is secondary lesions such as aspiration pneumonia or decubital ulcers. Antibiotics that can cause neuromuscular blockade and possibly exacerbate clinical signs such as aminoglycosides should be avoided and neurostimulants such as neostigmine should not be used. Good nursing care including the provision of a deep bed and a quiet environment are essential. Frequent turning of recumbent animals, nasogastric feeding and fluid support for animals with pharyngeal and lingual paralysis, frequent catheterization of the urinary bladder, application of ophthalmic ointments and ventilatory support may all be required.

If botulism is suspected to have been caused by ingestion of preformed toxin in feed, an alternate feed source should be provided while the origin is investigated. Potentially contaminated feeds.

Prognosis

A survival rate of 88% has been reported in foals with toxicoinfectious botulism that were provided with intensive nursing care (including mechanical ventilation and botulism antitoxin). However, this type of treatment is not available in all areas and is quite expensive. Without aggressive supportive care, the mortality rate is high, with death usually occurring 1-3 days after the onset of clinical signs.

The prognosis is variable in adult horses that have ingested pre-formed toxin, depending on the amount of toxin absorbed and the severity of clinical signs. Mildly affected animals may recover with minimal treatment while severely affected animals that become recumbent have a poor prognosis. The mortality rate has been reported to be as high as 90% in recumbent adult horses, with death occurring within hours of the appearance of signs. In animals that survive, complete recovery is most common. Development of full muscular strength takes weeks to months. Persistent tongue weakness not affecting the ability to eat has been reported.

Prevention

Type B toxoid is available and should be used in areas in which type B botulism is Vaccination is particularly important in areas where neonatal botulism occurs. Widespread vaccination of mares in certain high-risk areas has dramatically decreased the incidence of neonatal botulism. An initial series of three vaccinations a month apart followed by annual boosters has been recommended. Pregnant mares should receive a booster four weeks prior to foaling to ensure adequate antibody levels in colostrum. Type B vaccine only provides protection against type B toxin. There is no cross protection against type C toxin and type C toxoid is not licensed for use in North America.

Silage, haylage and other fermented feeds should not be fed to horses because of the risk of botulism.

Certification in Equine Rehabilitation and Performance Veterinary Medicine (CERPV)- almost done!

"PRE-HABILITATION" - as an evolving mindset to detect subtle body changes and minor physical adaptations so that we may initiate exercises and strategies before they escalate to lameness or discomfort.

Better understanding of "Why did this injury happen?" is a part of PRE-HABILITATION and helps us identify earlier intervention opportunities. Tense trigger points, performance stretching, fascia release, relieving restrictions, perturbation exercises, local vibration, re-engaging nerve pathways, and stabilization pads - these are all important to your horse's well-being that we should talk about.

Dr. Joyce has just 28 more online hours of videos and a challenging proctored final examination in January then he complete his Certification in Equine Rehabilitation and Performance Veterinary Medicine (CERPV) from Chi University in Florida. CERPV is a several-month course of study - a mix of many online modules and tests as well as two intense visits to Chi University for hands-on and laboratory application.

This visit involved even more on recognition of saddle fitting issues, performance horse shoeing updates, improving conditioning and rehab techniques, pain management, and maximizing injury healing to optimize outcome - all were further addressed this session.

This CERPV program is ONLY taught to veterinarians and is the most comprehensive and advanced program offered with specific emphasis on performance sport horse medicine and rehabilitation. Chi University offers the most instruction in injury prevention, the early diagnosis of performance deficits, how to correct them, and rehabilitation after injury.

After nearly a year of successfully working on real-world cases at NEC/NEFC, this certification further embodies TEVA's commitment to ensuring that our practical experiences match the latest in academics and science. We do this to provide your horses the greatest opportunity for prevention, a sustained riding career, and the fullest recovery from injury.

As a practice, we're seeking to raise the region's available on- and off-farm resources in lameness, rehabilitation, reconditioning, and preservation of soundness. TEVA sets the highest bar with qualified veterinarians to help you achieve your equestrian goals.

With our 2 board-certified Diplomates (Drs. Hyman and Coppelman) and Dr. Joyce soon "Certified in Equine Rehab and Performance" (CERPV), the TEVA team is more fully prepared to help you from initial injury to veterinary intervention and through all phases of recovery. Additionally, Drs. Hyman and Coppleman are Chi University certified graduates in Acupuncture (E is also Advanced), and Dr. Hyman is certified in Muscle Manipulation (Tui-Na massage). From piles to smiles.

Outdoor arena footing replaced and ready for riding. No more rocks and dirt and yuk (and removed the giant pole/fencing in the middle).

For the winter, we have a safe outdoor arena with good footing to ride and warm up area for our indoor's shows/clinics.

Larger adjustments will happen in the spring like adding fiber/adjusting the blend, reworking the base, replacing the fencing, adding sound system and lights, etc. The immediate goal was rideable and safe for horses.
