Topic: When to Recommend CSF After a Normal MRI
This week, Dr. Kendal Day (BVNS Rockville) presented a study on 553 dogs and 60 cats with neurological disease but normal MRI & CSF results:

📊 Only 3% had abnormal CSF (0 cats, 17 dogs)
🧠 CSF meaningfully changed diagnosis/treatment in just 1% of cases
🔍 Literature & experience show 20–25% abnormal CSF in certain MUE cases

Our takeaways:
✅ Still recommend CSF if the neuro exam is abnormal, MRI is inconclusive, or there are systemic illness signs
❌ Less likely to recommend CSF for patients with normal exams & presumed epilepsy
💡 This approach eases caregiver burden and focuses resources on rechecks, drug monitoring, and family support

Small shifts in protocol = big wins for patients & families.

Topic: Peritumoral Brain Edema (PTBE) in Meningioma

This week, Dr. Martin Young from BVNS Richmond led a Journal Club discussion on brain swelling (PTBE) in meningioma patients — a challenge he recently faced in surgery despite steroid therapy. This real-world case inspired him to dig deeper into better treatment options. He'll be presenting his findings this September at the Southeastern Veterinary Neurology Conference in Savannah, GA.

🔍 Key Insight:
Steroids are often ineffective against PTBE when VEGF-induced blood vessel leakage is severe — pointing to a need for alternative therapies.

💡 Why it matters:

PTBE affects ~50% of human meningioma cases and leads to seizures, complications, and longer hospital stays.

VEGF-A inhibitors like bevacizumab have shown promise in reducing PTBE in humans.

Dr. Young is exploring whether similar drugs could benefit veterinary patients — potentially improving outcomes in both surgical and medical management.

Topic: Re**al Trazodone for Safe Sedation in Dogs
This week, the BVNS neurology team, in collaboration with neurologists from SCAN in Florida, reviewed a study on rectally administered trazodone in dogs. The session was led by Dr. Divita, our Residency Advisor.

🔍 Key Insight:
Re**al trazodone provides a rapid, safe, and effective sedation option—ideal when oral or IV routes aren’t possible.

💡 Why it matters:
Moderate sedation achieved within 15 minutes, lasting up to 12 hours.
No adverse effects observed in study dogs.
Useful for seizure recovery, anxious patients, or during recovery from procedures—especially in brachycephalic or stressed dogs.

This week, Dr. Hollingsworth presented findings on DM in dogs—with and without concurrent disk disease.

🔍 Key Insights:
🧬 DM is linked to mutations in the SOD1 gene and leads to progressive pelvic limb paralysis.
🧠 MRI may appear normal, but 25%+ spinal cord compression from disk extrusions is common.
📉 Median survival: 13 months (DM alone) vs. 11 months (DM + IVDD)—no significant difference.
💊 Treatments like NSAIDs, steroids, or rehab didn’t improve survival time, but may support quality of life.

💡 Takeaway: This study sharpens our ability to guide owners with more accurate expectations and supportive care strategies.

💊 Keppra ER: Crushing (not splitting) provides faster seizure control—now part of our rescue protocols.
🐶 French Bulldogs: Vestibular signs? Recommend MRI—higher risk of brain infection due to skull anatomy.
🦴 Hemilaminectomy & DPP: One pelvic limb with deep pain = 80% success. We’re more optimistic about surgery in these cases.
😴 CKCS & COMS: Poor sleep = chronic pain. We’re now asking about nighttime restlessness to guide treatment.
🧠 Tethered Cord Syndrome: Dynamic MRI can uncover hidden causes of lumbar pain—14/14 dogs improved with surgery.
😟 Epilepsy & Anxiety: 84% of epileptic dogs show anxiety. We're prioritizing diagnosis and treatment.
