Please bring a copy of your pets vaccination history with you to your appointment, if it is not yet on file at The Animal Hospital. Due to the state law below we can no longer call and request medical records for your pets if you have not signed a release form with your veterinarian.

In accordance with state law, "Copies of records may be released to the client's designee or another veterinarian engaged by the client to examine or treat the patient upon submission of a signed Authorization for the Release of Medical Records form pursuant to KRS 321.185(3)."
