Prompt diagnosis of uterine inflammation (pyometra) is critical due to its high risk of mortality. In cases with co-occurring bacteremia (bacterial infection in the bloodstream), pre-treatment examinations must be even more thorough to increase the chances of survival for our beloved pets.

Let's explore what pyometra is.
Pyometra, or uterine inflammation in dogs, is a serious and life-threatening condition. It's commonly found in unspayed female dogs, particularly those that are older or have a history of using contraceptive medications 🥼.
The main cause is hormonal changes after the estrus cycle (heat). After being in heat, dogs experience elevated progesterone levels, which cause the uterine wall to thicken and produce more secretions in preparation for pregnancy. If pregnancy doesn't occur, this thickened uterine lining and increased secretions create a favorable environment for bacterial growth. The most common bacterium found is E. coli, which typically enters the uterus from the va**na or a**s when the uterus is in a state conducive to infection 🦠.
Pyometra is categorized into two types:
* Open Pyometra
* Closed Pyometra
Symptoms pet parents might observe at home (may vary depending on the type and stage of the disease):
* Lethargy, weakness, lack of energy
* Loss of appetite or reduced food intake
* Abnormal increase in water consumption and frequent urination (due to toxins from the infection affecting kidney function)
* Fever (may or may not be present)
* Vomiting, diarrhea
* Abdominal pain, distended or enlarged abdomen (especially in cases of closed pyometra)
* Discharge from the va**na, such as pus, pus mixed with blood, or dark blood – this is a clear symptom of open pyometra.
Surgery is considered the best and safest treatment method. Surgical removal of the entire uterus and ovaries permanently eliminates the source of infection and inflammation. Surgery should be performed as quickly as possible once diagnosed to prevent severe complications.

The best way to prevent pyometra is to spay your dog. Spaying your dog before their first heat cycle, or after their first heat cycle, significantly reduces the risk of pyometra and also helps prevent other diseases like mammary tumors.
Currently, the survival rate for pyometra cases is much higher than before. With improved technology and higher quality medications, treatment has advanced. Combined with pet parents who understand and actively participate in treatment planning with their veterinarians after surgery, dogs can recover faster and return to normal health more effectively.

เรามาดูกันครับว่า #มดลูกอักเสบ คืออะไร
มดลูกอักเสบในสุนัข หรือที่เรียกว่า " Pyometra " เป็นภาวะที่ร้ายแรงและอันตรายถึงชีวิต มักพบในเด็กๆผู้หญิงที่ยังไม่ได้ทำหมัน โดยเฉพาะอย่างยิ่งเด็กๆที่มีอายุมากขึ้น หรือมีประวัติการใช้ยาคุมกำเนิด 🥼
#สาเหตุหลักๆ คือการเปลี่ยนแปลงของฮอร์โมน หลังจากการเป็นสัด เด็กๆจะมีระดับฮอร์โมนโปรเจสเตอโรนที่สูงขึ้น ซึ่งจะทำให้ผนังมดลูกหนาตัวขึ้นและมีสารคัดหลั่งเพิ่มขึ้น เพื่อเตรียมพร้อมสำหรับการตั้งท้อง หากไม่เกิดการตั้งท้อง สภาพมดลูกที่หนาตัวและมีสารคัดหลั่งนี้จะเอื้อต่อการเจริญเติบโตของแบคทีเรีย แบคทีเรียที่พบบ่อยที่สุดคือ E. coli ซึ่งมักจะมาจากทางช่องคลอดหรือทวารหนัก เข้าสู่มดลูกที่อยู่ในสภาวะที่เอื้อต่อการติดเชื้อ 🦠
มดลูกอักเสบแบ่งออกเป็น 2 แบบ คือ
มดลูกอักเสบแบบเปิด และ มดลูกอักเสบแบบปิด
อาการที่ผู้ปกครองสังเกตได้ที่บ้าน (อาจแตกต่างกันไปขึ้นอยู่กับชนิดและระยะของโรค)
- ซึม, อ่อนเพลีย, ไม่ร่าเริง
- เบื่ออาหาร หรือ กินอาหารน้อยลง
- กินน้ำเยอะขึ้นผิดปกติและปัสสาวะบ่อยขึ้น (เนื่องจากสารพิษจากเชื้อโรคส่งผลต่อการทำงานของไต)
- มีไข้ (อาจพบหรือไม่พบก็ได้)
- อาเจียน ท้องเสีย
- ปวดท้อง, ท้องบวมหรือขยายใหญ่ขึ้น (โดยเฉพาะในชนิดมดลูกอักเสบแบบปิด)
- มีสารคัดหลั่งไหลออกจากช่องคลอด เช่น หนอง, หนองปนเลือด, เลือดสีคล้ำ ซึ่งเป็นอาการที่ชัดเจนของมดลูกอักเสบแบบเปิด
การผ่าตัด ถือว่าเป็นวิธีการรักษาที่ดีและปลอดภัยที่สุด การผ่าตัดนำมดลูกและรังไข่ทั้งหมดออก ซึ่งจะช่วยกำจัดแหล่งของเชื้อโรคและภาวะอักเสบได้อย่างถาวร การผ่าตัดควรทำโดยเร็วที่สุดเมื่อวินิจฉัยพบ เพื่อป้องกันภาวะแทรกซ้อนที่รุนแรง

วิธีที่ดีที่สุดในการป้องกันภาวะมดลูกอักเสบ คือ #การทำหมัน ให้เด็กๆครับ การทำหมันเด็กๆก่อนการเป็นสัดครั้งแรก หรือหลังการเป็นสัดครั้งแรก จะช่วยลดความเสี่ยงในการเกิดภาวะมดลูกอักเสบและยังช่วยป้องกันโรคอื่นๆ เช่น เนื้องอกเต้านมได้
