This is called Pharmacopuncture.  It’s a technique to administer drugs at specific acupuncture points. 

Pharmacopuncture may reduce opioid dose requirements while maintaining adequate analgesia.

An endoscopic injection catheter was advanced as far as possible, and 55 mL Coca-Cola mixed with 5 mL contrast was instilled directly into the affected intestinal segment.

This approach was pursued after endoscopic exploration revealed a jejunal trichobezoar lodged beyond the maximum reach of the scope (~140 cm). Multiple retrieval attempts using forceps, snares, irrigation, and suction were unsuccessful, and surgical intervention was declined.

Post-procedure imaging demonstrated diffuse gas distribution throughout the gastrointestinal tract, confirming delivery. Within 24 hours, contrast material had progressed to the colon with no evidence of persistent obstruction.

The patient recovered uneventfully, with resolution of clinical signs and successful passage of intestinal contents shortly thereafter.

This technique was extrapolated from human medicine, where Coca-Cola has been used to dissolve gastrointestinal bezoars—particularly phytobezoars—via nasogastric lavage or direct endoscopic infusion. Proposed mechanisms include its acidic pH (~2.6), mucolytic effects, and carbon dioxide content, which may help disrupt and fragment bezoar material.

𝐓𝐡𝐢𝐬 𝐰𝐚𝐬… 𝐮𝐧𝐛𝐞𝐥𝐢𝐞𝐯𝐚𝐛𝐥𝐞. 𝐇𝐞𝐚𝐝𝐚𝐜𝐡𝐞 𝐥𝐞𝐯𝐞𝐥 𝟏𝟎𝟎. 🐢🪡

In the middle of a busy surgery day, my colleague rushed in:
“Doctor — the tortoise swallowed a needle.”

Excuse me… what?!

And yes — there was thread protruding from the cloaca.

⁉️So many questions immediately:
• How did it even swallow that?
• How much effort did that take?
• It passed the stomach?
• It passed the small intestine too?
• Did it perforate anything?!

📌Plan A: Colonoscopy.
Follow the thread from the cloaca.
If we’re lucky — retrieve it endoscopically.
If not, or if perforation is suspected — we proceed to plastronotomy.

Anyone who has performed tortoise colonoscopy knows…
It is EXTREMELY challenging. So much f***l material. So much frustration. 😅

After copious flushing and carefully following the thread, we finally located the needle — lodged right at the ileocolic junction (between ileum and colon).

And incredibly… we managed to retrieve it endoscopically. ❤️
No perforation. No surgery needed.

Sometimes exotic medicine is chaos.
Sometimes it’s teamwork.
And sometimes… it’s pure luck.
