MAGIC IN THE WATER!
For someone who has worked mostly indoors seeing bioluminescence for the first time was like something out of a fantasy film!!
BIOLUMINESCENCE is cause by an algal bloom. This specific one was cause by Noctiluca scintillans, a dinoflagellate biolumenescent phyrrophyta.
A study by CMFRI shows that these are cause by sudden shift in wind velocity, change in temperature, increased nutrients or variation in rainfall.
Even though they slightly reduce the pH of the water and reduce BOD, they are neither harmful or beneficial for the marine ecosystem.

Its been a week since we left beautiful Mauritius. It feels good to be back home . Back to sounds ,smells and sights we are familiar with but we must admit we sorely miss Mauritius. It will always be a part of our hearts.
The amazing, crazy and absolutely beautiful people that we met made our stay so special. They opened their homes and hearts to us.
Two years flew by so quick, from long hard days at work, chilling on the beach, long drives to no where, midnight emergencies, barbques and endless bottles of beer , you'll have been by our side. Never for once allowing us to feel homesick. We shared tears, abuses and a whole lot of laughter. It gave us so much joy to teach and see the young vet techs and vets learn and grow! Kamilla's lunchs and motherly love were the best!! We also saw ourselves grow personally and professionally.
A HUGE MERCI BEAUCOUP to all the people who crossed our paths and added a little more sparkle to it!! 🥰😍🤩(I have missed a few people cause I couldn't find pics 😪)

We actually prefer to do this surgery over Scapulectomy unless there is severe muscle loss or presence of metastatic cancer which makes removal of pre and post LN's essential. We havnt encountered muscle loss or pressure sore post op in most of our cases but if you have a difference of opinion lets discuss.

When doing any orthopedic surgery we always do a CRI with Ketamine-lidocaine something that introduced us to which gives a superb pain relief during and post op. If u want to know how we do it just leave a comment below.

So just to admit we don't have an electrocautery unit and that makes the surgery look a bit messy. Over the years we have figured out actually knowing a bit of anatomy and having a rough idea on the set of muscles u encounter and dissecting at point of origin/insertion drastically reduce bloodloss.

This is something that we have practiced and recommend to everyone we teach; is after amputation to open and learn anatomy and possibly do practice orthopedic procedure on it so it doesn't ever go wasted.

This is one surgery where if you apply Halsted principles- asepsis, haemostasis, gentle tissue handling, anatomic dissection, apposition and avoiding dead space will have an painless recovery (mobility in 12 hours) and no requirement of post op antibiotics. Also we are very liberal with out Bupivacaine/lidocaine splash throughout the surgery which add to analgesia.

Skin grafts (2 of 2) Sweat or Swear & Aftercare:

3. Placing graft on graft bed: Use 3 -0 PDS/Prolene or staples and carefully suture graft to wound edges using simple interrupted sutures.
4. BANDAGING: Most important part of graft 'take' has a lot to do with drainage & immobilization post op. We did a nonadherent absorbent layer bandage for first 10 days post op with bandage changes every 3rd day. Avoiding fluid accumulation can be achieved by having a closed suction or vacuum assisted bandaging also.
DISCOLORATION PATTERNS: This is how a normal pattern goes with graft, pale till day 1-3, blue till day 3-5, reddish on day 5; so don't panic!! Edema happens for 72hrs which is normal so either provide Drain or absorbent bandage
🚩 would be necrosis, infection and nonadhesions but don't worry just hold on and continue with bandaging... dont have to declare failure so soon. Resist changing bandages daily!

FUN FACTS: Remember to check the direction of the hair and possibly mark it when taking a graft from donor site else it looks very stupid when healed 😂.

Skin grafts (1 of 2) Preperation and Gratifying Part:
I have been in love with this surgery ever since Dr. Nitin introduced me to it who has been a friend and inspiration to me over the years. We have done quite a few grafts in the past few years and have either had them heal beautifully or fail miserably.
Grafts can be either full or partial thickness skin graft, belonging to the same individual/ species or an entirely different species for eg. Tilapia skin grafts (definitely on our to do list).
Grafts can be done on wounds on sites like the limbs or trunk where closures with flaps or skin apposition are difficult and movement is limited.
The procedure basically involves:-
1. Preparation of the graft site: Wound with good granulation tissue free of infection are ideal. We initially treated our cat with alternate day moist bandaging with paraffin infused gauze till we got the desired wound site (shelter tip you can always use liquid paraffin or even vaseline smeared on a gauze instead of commercial paraffin infused gauzes).
During surgery scrape the granulation tissue lightly to get bleeding edges & then keep the area moist with a wet gauze till graft is prepared.
We always pre measure the wound and mark out the graft using a sterile marker on a sterile plastic or paper of our surgical pouch.
2. Preparing the graft: We have always taken the graft from the flank region but any place with excess skin like lateral thorac is fine. Always Harvest the graft with exact dimensions or a little more.
To make partial thickness grafts we remove all the subcutaneous fat carefully from the skin and having a sexy metzenbaum for this is a must 💃🏻 We hold the graft on a cardboard covered with sterile drape using 24 G needles to keep it stretched. The graft should also be kept moist with sterile RL. Make several parallel stab incisions on the graft which prevents fluid buildup between wound & graft.

Feline lymphoma....Chance/ Soul crushing diagnosis but also lesson on having differential diagnosis:
Pounette,10 year old female neutered cat presented with month long weightloss and a peculiar plantigrade gait. She was being treated for her plantigrade gait at a private clinic but had been unresponsive, so the owners brought her in for a second opinion.
Her physical exam showed BCS 1/5, tachycardia 200+, glucose 6.5 and enlargment in submandibular region.
Frankly our differentials initially included diabetes (plantigrade) or hyperthyroidism (tachycardia, weight loss) and not lymphoma initially. Her CBC showed no abnormalities, serum showed elevated liver values and normal thyroid values which was very confusing.
A bit of read up later we kind of narrowed it down to either lymphoma or a metastatic neoplasm (leukemia or mast cells).
When she came about 10 days later she had developed icteric and had a mass just caudal to the hepatomegaly. Lucky for us the owner agreed on post mortem after euthanasia.
Since coming to Mauritius this is our 4th animal that we have euthanised past diagnosis of lymphoma but first that we could do a post mortem and learn more from.
Post mortem showed enlarged cistern which had adhesions to the pancreas and gall bladder, enlarged sublumbar/ mediastinal and retropharyngeal LN.
Lessons learnt would be diagnosticwise we could have done more like retroviral assays/ flow cytometry to classify the class and stages of lymphoma. Also at one point in next 5 years say we would like to be able to procure Doxorubicin/ Cyclophosphamide needed apart from vincristine and pred. and be actually able to treat a case after breaking our heads to diagnose it.

Hey guys need a huge favour (3&4/4):
We are planning on going back to India in Sept and would like to find a forever home to our fosters. This is our band of broken angels whom we have been fostering since they came in PAWS. Everyone has a story and a character that makes them truely special to us and it would break both, our heart and their soul if we don't find a home for them and they have to go back in kennels awaiting a family.

Meows 3 year old female actually, belonged to the previous owner for our house and was abandoned when they moved. She has been kind enough to let us share some space. Minds her own thing, has extreme cat behaviour when it comes to bossing around dogs and humans, loves to have conversations or just cuddle up and sleep next to you the whole day.
Sheela rescued by preety, 2 years old came to us with an extreme fracture on her humerus, broken pelvis, and old fracture on her hind leg. We honestly tried for 4 hours to fix but couldnt. We ended up doing an amputation for her and felt very guilty worried about how her rehab would go with her other fractures hence she ended up at our home that night. For 2 weeks she would only lie down and just move about to do her business... it was one of our then fostered puppies Annie who persisted and got her to play and move around. Now 3 months down she is a bit shy towards strangers but extremely playful and loves every new puppy that we got to our house afterwards.

During our time in PAWS we have fostered many dogs and cats and did end up finding them forever homes. Now with time only these 4 remain and we want your help to share it to as any friends and family as possible and help them get to their.

WHEN THE DOCTOR BECOMES THE PATIENT

The phrase " you never know what will happen next" never felt more true during our vacation (which was looong awaited) .
A seemingly simple tour of a secret beach became quite an adventure.
An unexpected fall resulting in a calcaneous bone fracture , rescue by police and firemen, spending a whole evening at the hospital, police station and next day at airport to try and get a flight back will make our trip to the stunning island of Rodrigues unforgettable.

Ishan being ishan after turning our day upside down took everything with a pinch of salt and was a trooper the whole time (except when it came time for injections).
Funny how we do countless surgeries but when Ishan was in that position he was geared up to run out in patient gown from the operation theatre...
After 2 lag screws through his heel and 3 days in hospital he is back home in one piece and healing! Ever so gratefull for the police department, fire rescue, staff , doctors and staff at Rodrigues and SSRN who convinced, coerced and kept the child busy with cool equipment in the operation theatre to put him under and fix him.

This is the one time we did not insist on castration when patient was under anaesthesia 😉🤣

Squamous cell carcinoma in cats:
This is our third cat (2 on ear tips/ 1 on nasal planum) that we have encountered over past 2 years. These are malignant cancers especially in a white cat with light haired areas.
Patients often present with a non healing wounds on extremities and a highly frustrated owner who hasn't gotten a proper diagnosis for his pet from any vet.

The wounds are plaque covered, ulcerated and often mistaken to be a wound in granulation stage of healing.

Impression cytology is explained in the slide photo which is highly diagnostic in most cases.

All though in most cases the owner chooses euthanasia, its of paramount importance that we have a diagnosis which makes the decision easier.

Treatment plan would include a wide surgical incision for ex. Removing the nasal planum or ear tips which mind you are always temporary solutions to a soon to be bigger problem.
Apart from surgical various chemotherpy can now be used to delay and improve life span/quality of life, sadly not in mauritius yet....

Partner came to us with a couple who promised to take him to Europe (ghosted us🙄) and had just rescued him from an owner who was ill treating him. He came as skin and bones, malformed hind limbs, wound on the elbow and untrusting of even a leaf. Over the period of his wound treatment we took him home and tried to get him more friendly and adoptable. He is the most playfull of our lot, very vocal in all discussions, loves an open ground and now also a car. His disability hasn't stopped him from chasing ball+ and competing a normal four-legged dog in a race.

During our time in PAWS we have fostered many dogs and cats and did end up finding them forever homes. Now with time only these 4 remain and we want your help to share it to as any friends and family as possible and help them get to their.
