Pain Isn’t Always Where the Problem Is

One of the biggest misconceptions about pain is that it always tells you where the problem is.

A recent patient, Monty, is a great example of this.

Monty was referred by his veterinary surgeon following a diagnosis of left shoulder tendinitis. On initial assessment, there was obvious tightness throughout the left shoulder and thoracic sling muscles, along with a mild left forelimb lameness.

But the full assessment revealed much more.

Months of protecting that sore shoulder had changed the way Monty moved. He had developed significant muscle tension and hypertonicity through his right hindlimb, particularly the gracilis muscle, with reduced muscle flexibility and discomfort on palpation. These weren’t primary injuries—they were secondary compensatory changes caused by his altered gait.

This is something I see regularly in dogs with injuries.

If we only focused treatment on Monty’s shoulder, we would have missed the secondary restrictions that had developed elsewhere.

This is why every assessment I perform looks at the whole dog, their posture, gait, movement patterns, muscle tone and fascial restrictions, not just the area where they’re showing pain.

Pain is the symptom.

Understanding why the body has adapted is where meaningful rehabilitation begins.

Monty is now progressing well with a rehabilitation programme designed to address both the original injury and the compensatory changes that developed over time.

Feeling the Flow: A Modern Perspective on “Chi”

For centuries, practitioners of Traditional Chinese Medicine have described a vital force—known as Qi (Chi)—that flows throughout the body, influencing health, movement, and well-being. Although modern science uses different terminology, many of the sensations traditionally associated with chi—warmth, ease, connection, release, and fluid movement—are familiar to anyone who has experienced effective bodywork or effortless movement.

Rather than asking whether chi exists as a measurable substance, it may be more useful to ask whether traditional and modern medicine are describing the same experience using different languages.

The Body Is Constantly Sensing Itself

The body is continuously gathering information about itself.

Sensory receptors in the skin, muscles, fascia, tendons, ligaments, joints, and internal organs constantly send information to the nervous system about touch, pressure, stretch, tension, movement, body position, temperature, and the body’s internal state.

Two sensory systems play particularly important roles:

* Proprioception — awareness of body position and movement.
* Interoception — awareness of internal sensations such as breathing, heartbeat, muscle tension, temperature, and other physiological processes.

Together, these systems allow the brain to build an ongoing internal map of the body. When that map becomes clearer and more organized, movement often feels smoother, more coordinated, and more connected.

How Massage and Fascial Therapy Change the Body

Massage and fascial therapy influence multiple physiological systems at the same time.

By reducing excessive muscle guarding and calming sympathetic nervous system activity, they decrease the background “noise” created by pain, stress, and protective tension. As unnecessary tension subsides, more subtle sensations become easier to perceive.

Manual therapy also increases local circulation and encourages the movement of blood, lymph, and tissue fluids. As tissues warm and become better hydrated, they often feel softer, more responsive, and easier to move.

At the same time, massage improves movement between fascial layers. Fascia forms a continuous network throughout the body, transmitting force while allowing neighboring tissues to glide over one another. When this glide improves, movement often feels less restricted and more continuous.

Touch itself is equally important.

Every stroke, stretch, and change in pressure stimulates mechanoreceptors throughout the skin and fascia, providing the nervous system with updated information about the body’s mechanical state. This continual sensory input refines proprioception and interoception, improving the brain’s awareness of the body.

Why Movement Amplifies the Feeling of Flow

Massage prepares the body, but movement allows those changes to be experienced.

With every step, muscles contract and relax, fascia stretches and recoils, joints move through their range of motion, and forces are distributed throughout the body. At the same time, circulation increases, tissues generate heat, and fluids are redistributed through the extracellular matrix that surrounds cells and supports the fascial network.

As tissue temperature rises, the viscosity of the extracellular matrix decreases, allowing fascial layers to slide more freely over one another. Repeated loading and unloading further encourage this glide while continually refreshing the sensory information reaching the nervous system.

Movement also creates an ongoing feedback loop. Every change in posture, tension, and load is immediately detected by sensory receptors, allowing the brain to continuously refine coordination and body awareness.

The result is often a growing sense of continuity throughout the body. Instead of feeling like separate muscles and joints working independently, movement begins to feel integrated, effortless, and fluid.

Why Some Forms of Sensory Input Feel Especially Powerful

The nervous system naturally gives greater attention to sensory information that is new, widespread, or continuously changing.

Massage, fascial therapy, movement, elastic taping, compression, stretching, and other forms of tactile stimulation all provide this type of rich sensory input. Because these inputs often involve multiple body regions simultaneously, they help the brain perceive the body as a more connected whole.

As sensory information becomes clearer, movement pathways become more refined, coordination improves, and internal awareness increases. Many people experience this as a greater sense of “flow”—a sensation that may correspond to what Traditional Chinese Medicine describes as the free movement of chi.

A Physiological View of Chi

Traditional Chinese Medicine describes the free movement of qi through the body. Modern physiology describes coordinated movement, rich sensory feedback, efficient force transmission through fascia, healthy circulation, fluid dynamics, and balanced nervous system function.

These explanations are not necessarily in conflict. They may simply emphasize different aspects of the same experience.

When unnecessary tension decreases, fascial tissues glide more freely, circulation improves, movement becomes coordinated, and the nervous system receives clear sensory information, people commonly report feeling:

* Warmer
* Lighter
* More connected
* Less restricted
* Easier to move
* More aware of their body

These are many of the same qualities historically attributed to the free flow of chi.

Final Thought

Massage, fascial therapy, movement, and other forms of meaningful sensory input improve the body’s mechanical function while enhancing the nervous system’s ability to perceive what is already happening.

From a modern scientific perspective, the experience of chi may emerge from the combined effects of improved circulation, more efficient fascial movement, balanced nervous system activity, and heightened sensory awareness.

Whether we describe that experience as optimized physiology or the free flow of chi, the result is remarkably similar: a body that feels warmer, more connected, more coordinated, and more alive from within.

The Emerging Science of Fascia and Expanding Our Understanding

Modern research has shown that fascia is far more than connective tissue. It forms a continuous body-wide network that senses mechanical forces, communicates with the nervous system, distributes load, stores and returns elastic energy, and adapts to training, injury, and rehabilitation.

Some researchers have even proposed that the fascial network—and perhaps its water-rich extracellular matrix—functions as a highly responsive biological communication system, or even an “antenna” for mechanical information. While this hypothesis remains under active investigation, it reflects just how rapidly our understanding of fascia is evolving.

As scientists continue to study this remarkable tissue, fascia is emerging as one of the most important integrative systems in the horse’s body, influencing movement, coordination, performance, and recovery in ways that were unimaginable only a few decades ago.

Fascia is a Sensory Hub

Many fascial tissues contain substantially more sensory nerve endings than adjacent muscle, including mechanoreceptors that detect tension, pressure, stretch, vibration, and movement. These receptors continuously send information to the nervous system about body position, loading, and movement, contributing to proprioception and motor control.

Rather than acting as an inert wrapping around muscles, fascia functions as an active sensory interface between the body and the nervous system.

Movement begins before muscles contract.

Emerging research suggests that the fascial network is continually maintained under a degree of resting tension, sometimes referred to as pre-stress. This tension may help pre-position and pre-load the body, allowing movement to begin more efficiently.

Instead of movement being initiated solely by muscle contraction, fascia participates in preparing the body for movement while simultaneously providing continuous sensory feedback to the nervous system.

Fascia, it seems, contributes to movement in ways that extend beyond far beyond simply connecting muscles together.

Fascia forms a continuous three-dimensional connective tissue network that links muscles, tendons, ligaments, bones, and organs.

Through this network, fascial pathways distribute force is across multiple regions of the body.

A well-conditioned fascial network improves the efficiency of force transmission, coordination, and movement while also contributing to elastic energy storage and return during locomotion.

Fascia is Dynamic and Adaptable

Fascial tissues constantly respond to movement and loading.

Their mechanical properties change with exercise, training, injury, hydration, age, and recovery stage. Fascia continually remodels in response to the demands placed upon it.

This adaptability helps explain why movement quality, conditioning, recovery, and bodywork can all influence how the body functions.

Fascia Also Has Unique Electrical Properties

Collagen-rich fascial tissues exhibit piezoelectric properties, meaning that mechanical loading can generate tiny electrical potentials within the tissue.

These signals are thought to contribute to cellular communication, tissue adaptation, and remodeling. The water-rich extracellular matrix surrounding fascial tissues also plays an important role in their mechanical and electrical behavior, making this an active area of research in mechanobiology.

An Expanding Understanding

Modern fascia research increasingly recognizes fascia as one of the body’s primary integrative tissues, linking sensation, movement, force transmission, adaptation, and communication.
